Requirements
  • A bachelor's degree in social work, social services, counseling, business administration, or a related field from an accredited institution, or at least five years of case management experience in lieu of a bachelor's degree, or work experience acceptable to the agency.
  • Three years of experience in vocational rehabilitation, case management, or a related field.
Responsibilities
  • Carry a caseload of approximately 50 unemployed and unenrolled program participants.
  • Conduct at least 25 participant interactions per week, including bi-weekly coaching sessions and/or weekly non-compliance or re-engagement meetings.
  • Coordinate and develop relationships with external providers, including secondary service providers, district agency providers, schools, employers, and community providers, as well as teammates, to ensure participants make meaningful progress toward employment.
  • Review DHS-administered assessments, develop and amend participants’ Detailed Individual Responsibility Plans (DIRPs), and help participants overcome barriers to program engagement and participation.
  • Develop trusted working relationships with assigned TANF participants and monitor engagement and progress through the participants’ first year of employment.
  • Provide counseling, intervention, program action planning, and program action delivery.
  • Facilitate participants’ successful use of job-readiness training, rapid job searching, job-retention services, and/or education services.
  • Document activities, participation, compliance, and employment progress in Electronic Case Management systems.
  • Guide participants through service planning and delivery using a learn-by-doing process and emphasize urgency and ownership.
  • Achieve an 80% Work Participation Rate on the caseload by collecting, verifying, and processing timesheets and/or paystubs for the entire caseload weekly.
  • Complete weekly non-compliance attempts for the 20% of the caseload not meeting the Work Participation Rate, including phone calls, emails, letters, and home visits; re-engage responsive participants or confirm sanction status to DHS for those who have not responded.
  • Coordinate activities outlined in participants’ Detailed Individual Responsibility Plans and assist participants in reporting work participation.
  • Take responsibility for participant engagement and work with staff to identify appropriate activities, resources, and supports.
  • Meet or exceed performance targets and key performance indicators, including job placements, employment retention, school completions, case closure due to earnings, customer satisfaction, and audit scores.
  • Maintain 100% accuracy in internal data systems, including timely recording of employer activity, jobseeker interactions, and placement outcomes.
  • Work with internal teams to maintain service consistency and accountability and improve service efficiency.
  • Capture and record employment information, then work with customers to maintain and obtain job promotions and/or salary increases.
  • Verify and report customer milestone achievements to DHS.
  • Assist in compiling data for reports, including customer timesheets and placement and retention reports.
  • Observe policies and procedures and attend required training and certifications as scheduled.
