Requirements
  • A Doctor of Pharmacy degree is required.
  • A New York State Pharmacy License is required upon hire.
  • Board certification in the most relevant specialty area is required within two years.
  • Equivalent combinations of education and experience may be considered.
Responsibilities
  • Provide comprehensive clinical pharmacy services for a specialized area of pharmacy practice.
  • Participate in interdisciplinary patient care team rounds and provide consultative input on safe and effective drug therapy.
  • Manage established clinical pharmacy programs or services and monitor assigned patient populations for clinical progress, including renal and hepatic function, culture results, and other applicable laboratory tests.
  • Recommend therapy adjustments as necessary, provide patient education and discharge counseling, authorize and verify orders, and supervise drug distribution when appropriate and required.
  • Document clinical pharmacy activities in the medical record or other systems.
  • Collaborate with clinicians and healthcare providers to implement and maintain disease or medication management programs and clinical pharmacy services.
  • Participate in the development of University-wide medication use management programs, including clinical guidelines, critical pathways, disease management, and drug use programs.
  • Provide clinical leadership in the specialty area by mentoring, coaching, and serving as a role model for pharmacists in the department.
  • Direct and supervise technicians' work to ensure appropriate workflow as required.
  • Conduct quality assurance, medication use evaluation, or other research to document the value of clinical pharmacy services.
  • Participate in departmental quality assurance processes, including reporting and following up on medication incidents, reconciling medication administration records, and supporting cost-savings initiatives.
  • Serve as a preceptor for pharmacy residency training programs and contribute to student training experiences for introductory and advanced pharmacy practice rotations.
  • Provide didactic lectures, case studies, and other interactive teaching programs to pharmacy students, pharmacy residents, pharmacists, nursing staff, medical and surgical residents, and allied health students.
  • Resolve problems through clear and concise communication with pertinent staff, escalating issues when solutions exceed expertise or authority.
  • Identify patient-care problems, make reasonable attempts to resolve them, and communicate progress to the next shift and relevant interdisciplinary teams.
  • Revise hospital or departmental policies, procedures, or treatment guidelines when needed to resolve patient-care problems.
  • Maintain active membership and participate in local, regional, and national professional organizations in clinical pharmacy or specialty practice.
  • Present clinical education or research results at local, regional, and national meetings.
  • Provide continuing professional education to peers in the community, region, and nationally.
  • Serve on departmental, hospital, or medical center committees and task forces.
  • Perform other duties as assigned.
Desired Qualifications
  • Specialty residency training is preferred.
  • New resident graduates or clinical specialists in their first three to five years of practice are preferred.
