Requirements
  • The candidate must possess, at minimum, a nursing degree from an accredited college or university or be a graduate of an approved Licensed Practical Nurse/Licensed Vocational Nurse program.
  • The candidate must possess a current, unencumbered, active license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse in the state.
  • The candidate must be able to read, write, speak, and understand English and comprehend technical procedures and policy and procedure manuals.
  • The candidate must be able to apply fractions, percentages, ratios, and proportions to practical situations.
  • The candidate must demonstrate knowledge and skills necessary to provide care appropriate to the age-related needs of residents.

Responsibilities
  • Direct the day-to-day functions of nursing assistants and ensure nursing personnel comply with facility policies, procedures, job descriptions, and applicable regulations.
  • Review nursing policies, procedure manuals, job descriptions, and related materials and recommend revisions as needed.
  • Plan shift services, programs, and activities with assigned nursing and support personnel.
  • Prepare written and oral reports and recommendations concerning shift activities.
  • Coordinate nursing services with other resident services to maintain each resident's total care regimen.
  • Participate in the nursing service quality assurance program and facility surveys conducted by authorized government agencies.
  • Review and update resident discharge plans and assist with planning the nursing services portion of those plans.
  • Admit, transfer, and discharge residents and complete related documentation, including accident and incident reports.
  • Complete medical forms, reports, evaluations, studies, charting, census reports, charge slips, and other required administrative records.
  • Protect assigned user credentials and resident protected health information and report suspected unauthorized access or disclosure.
  • Complete and file required records and charts for resident admission, transfer, and discharge.
  • Receive and transcribe physician telephone orders and record physician orders, progress notes, diets, treatments, and care plans in resident records.
  • Chart nurses' notes descriptively to reflect care provided and the resident's response, sign and date medical-record entries, and report charting or order discrepancies.
  • Prepare and administer physician-ordered medications and treatments after verifying resident identity.
  • Maintain medication, narcotic, floor-stock, supply, and equipment levels; order prescribed medications and supplies as needed.
  • Review medication cards for accuracy and completeness, notify physicians of automatic stop orders, and dispose of drugs and narcotics according to procedure.
  • Participate in employee performance evaluations, determine shift staffing requirements, and recommend staffing actions to the Nurse Supervisor.
  • Develop work assignments, provide shift leadership, conduct rounds, monitor performance against nursing standards, and report problem areas.
  • Inform the Nurse Supervisor of staffing needs and absentee call-ins and review the department workforce for improvement recommendations.
  • Ensure nursing assistants are enrolled in or have graduated from approved training programs and that nurse aide trainees receive direct supervision from a licensed nurse.
  • Meet regularly with nursing personnel to identify and correct problem areas and improve services.
  • Maintain working relationships across departments, review staff complaints and grievances, and report or administer disciplinary matters according to facility procedures.
  • Receive and give nursing reports at the beginning and end of shift duty hours.
  • Report occupational exposures, suspected fraud, and information-security issues according to facility procedures.
  • Inform nursing personnel about new admissions, prepare rooms, greet and orient residents and families, and escort residents as necessary.
  • Make physician rounds, arrange ordered diagnostic and therapeutic services, consult physicians about resident care, and review resident charts for treatments, medications, and diets.
  • Implement nursing objectives and standards and check that prescribed treatments are properly administered and resident status is evaluated.
  • Coordinate social and activity programs with nursing schedules and notify physicians and next of kin about accidents, incidents, or changes in resident condition.
  • Carry out restorative and rehabilitative programs, including self-help and care activities.
  • Provide professional nursing services including catheterization, tube feedings, suctioning, dressing changes, colostomy and drainage-bag care, specimen collection, massages, range-of-motion exercises, and care of dying residents.
  • Use restraints when necessary and according to established policies and procedures.
  • Take and record temperature, pulse, respiration, blood pressure, and other required observations and monitor seriously ill residents.
  • Check food brought by residents' families or visitors against dietary allowances and report problems to nursing and dietary supervisors.
  • Ensure direct care follows each resident's care plan and wishes and check frequently on residents unable to call for help.
  • Arrange transportation and escorts for discharged residents, inform families of resident deaths, contact funeral homes when requested, and ensure postmortem procedures are followed.
  • Develop, plan, conduct, and schedule in-service training and maintain an effective orientation program for new employees.
  • Assist with standardizing work methods, training personnel on blood and body-fluid exposure tasks, and planning clinical supervision for nurse aide trainees.
  • Attend outside training, annual facility in-service programs, advance-directive training, and continuing education required to maintain professional licensure.
  • Monitor compliance with safety regulations, dress codes, infection-control procedures, hand hygiene, and use of personal protective equipment.
  • Maintain clean, safe, and sanitary nursing work areas, resident-care rooms, and treatment areas.
  • Ensure personnel participate safely in fire-safety and disaster-preparedness drills and use required safety equipment when lifting or moving residents.
  • Participate in development, implementation, and maintenance of infection-control and hazardous-condition reporting procedures.
  • Recommend unit equipment and supply needs and maintain adequate medication, medical-supply, and equipment stock.
  • Ensure only trained and authorized personnel operate nursing equipment safely and that supplies are used efficiently.
  • Maintain appropriate material safety data sheets for hazardous chemicals used by the nursing service department.
  • Review resident care plans daily, report needed changes, and ensure nursing notes document care-plan adherence.
  • Review resident care plans for appropriate goals, problems, approaches, and revisions and ensure certified nursing assistants understand and use them.
  • Assist with Minimum Data Set assessments, Resident Assessment Protocols, and triggers.
  • Provide requested data to the Quality Assurance and Assessment Committee.
  • Follow facility policies related to emergency call-back, continuing education, infectious exposures, hazardous chemicals, and resident safety.

PACS Group has appointed Carey P. Hendrickson as chief financial officer, effective 27 April 2026. Hendrickson brings nearly four decades of financial leadership across healthcare, senior living and media sectors. PACS Group has announced that co-founder and chief financial officer Mark Hancock will retire on 30 June 2026. He will continue serving on the company's board of directors as vice chairman. Hancock co-founded PACS in 2013 with Jason Murray, starting with two post-acute care facilities in San Diego.
