Requirements
- An active Michigan PTA license in good standing.
- Reliable transportation, a valid driver's license, and auto insurance.
- Competency in core home health physical therapy assistant scope, including safety-first mobility work; transfers; gait training; balance and fall prevention; assistive device recommendations; strengthening; functional mobility; home safety assessment and modification education; and patient/caregiver coaching with practical home programs.
- Ability to carry or transport basic therapy equipment as needed.
Responsibilities
- Deliver skilled physical therapy assistant visits in the home under the supervising physical therapist’s plan of care.
- Execute treatment plans focused on functional mobility and measurable progress, including gait and balance, transfers, strengthening, functional mobility, home safety, and fall prevention.
- Set and track objective progress within the physical therapy assistant scope and communicate updates and concerns to the supervising physical therapist.
- Coordinate multidisciplinary care by escalating changes in condition, aligning on orders, and keeping the care team moving.
- Coach patients and caregivers with practical home programs.
- Document promptly and thoroughly in a defensible, efficient, and consistent manner.
- Carry a full, high-quality caseload, ramping to approximately 30 productivity points per week at target visit quality.
- Drive patient outcomes, including low avoidable-hospitalization rates and strong patient-experience scores, and participate in case conferences and team huddles to improve quality.
- Represent the organization to referral sources through responsive, excellent patient care.
- Help set clinical, documentation, and care-delivery standards as the team grows.
