Requirements
  • Working toward an Associate’s degree in Human Services or a Bachelor’s degree in Social Work.
  • A minimum of one year of experience.
  • Completion of Vroon Vandenburg High Fidelity Wraparound Training.
  • Clinical skills in systemic family therapy, crisis intervention, family education, and linking and advocacy.
  • A valid driver’s license with an approved driving record of fewer than six points, personal transportation, and insurance if required to drive on behalf of the agency.
  • Ability to perform responsibilities with initiative and independent judgment.
  • Awareness, sensitivity, and skills for working professionally with diverse individuals, groups, and communities.
  • Clear verbal, nonverbal, and written communication in a professional context.
  • Ability to apply professional and ethical concepts within the scope of work and follow agency policies and procedures.
  • Ability to function effectively as a member of a professional team.
  • Ability to recognize problems, identify root causes, and make timely decisions.
Responsibilities
  • Learn about care coordination and provide care coordination services to clients and families identified by the CME Catchment Area.
  • Coordinate formal and informal services for children and families according to the High-Fidelity Wraparound model.
  • Engage families in the Wraparound model, plan and monitor service provision, and collaborate with involved partners.
  • Observe and learn intensive care coordination and case management services for children and young adults with severe and persistent mental health needs.
  • Shadow Care Coordinators in developing and maintaining service relationships with clients, families, and caregivers.
  • Participate in structured High-Fidelity Wraparound service planning and care coordination activities, including supported involvement in care plan development and monitoring.
  • Observe and assist Care Coordinators under supervision with referrals and linkage to appropriate community resources.
  • Learn documentation standards and confidentiality requirements for legal, financial, and clinical records.
  • Participate in training related to cultural competency, trauma-informed care, and program requirements.
  • Observe the identification and escalation of urgent, unsafe, and crisis situations to supervisors.
  • Attend agency, team, and clinical meetings, mandatory trainings, and treatment coordination activities.
  • Observe and support discharge planning and transition activities as appropriate.
  • Demonstrate positive leadership and promote a team-based work environment and culture of belonging.

Bellefaire JCB receives $522K grant to fight youth violence

The Semi J. and Ruth W. Begun Foundation, a supporting foundation of the Jewish Federation of Cleveland, recently awarded a $522,000 grant to Bellefaire JCB to create the Begun Center for Youth and Family Resiliency, which aims to reduce youth violence through social skills education and conflict resolution training, according to a news release.
