Requirements
  • A current registered nurse license in the state of employment, or eligibility to obtain one, is required.
  • At least one year of nursing experience is required.
  • A valid driver's license, automobile insurance, and reliable transportation are required.
  • Current cardiopulmonary resuscitation certification is required.
Responsibilities
  • Report directly to the Administrator, Executive Director, or Patient Care Manager Senior.
  • Provide direct patient care and support after normal business hours.
  • Serve as the primary liaison between patients, families, physicians, and the hospice team during the shift.
  • Respond to urgent calls, patient visits, and symptom management needs.
  • Evaluate patient and family needs to guide clinical decisions and implement the plan of care.
  • Support and collaborate with an interdisciplinary group including licensed practical nurses/licensed vocational nurses, certified nursing assistants, and other support staff.
  • Document clinical visits and communications accurately.
  • Participate in quality assessment and performance improvement programs.
  • Work independently while ensuring continuity of care and coordinating with the daytime care team.
Desired Qualifications
  • At least one year of hospice, home health, oncology, or palliative care experience is preferred.
  • Familiarity with symptom management, triage, and after-hours home-based care is preferred.
  • Prior experience in on-call or overnight nursing roles is preferred.
  • Strong documentation and electronic medical records skills are preferred.
