  • A current Kansas Licensed Practical Nurse license in good standing is required.
  • A valid driver's license with a clean driving record is required.
  • Current automobile insurance is required.
  • Reliable transportation and willingness to travel throughout the community are required.
  • The ability to pass a vehicle inspection or provide proof of an inspection completed within the past year is required.
  • The role requires nursing assessment, critical thinking, crisis response, documentation, independent work, and collaboration with a multidisciplinary team.
Responsibilities
  • Provide nursing care in clients' homes and other community settings, including nursing assessments, medication administration, medication education, and health monitoring.
  • Coordinate care with psychiatrists, primary care providers, hospitals, pharmacies, and community partners to ensure continuity of care.
  • Advocate for clients by helping them access medical, behavioral health, and community resources.
  • Educate clients and their families about medications, physical health, mental health conditions, and wellness strategies.
  • Assist with crisis intervention and emergency response by coordinating with crisis services, emergency responders, supervisors, and medical providers.
  • Participate in ACT team meetings and collaborate on individualized treatment plans.
  • Help clients develop skills to improve independence and manage emotional distress.
  • Build trusting, recovery-focused relationships with clients and their families.
  • Complete documentation accurately and in a timely manner.
  • Transport clients and provide services throughout the community as needed.
Desired Qualifications
  • Experience working in behavioral health, community mental health, or psychiatric nursing.
  • Experience supporting individuals with severe and persistent mental illness.
  • Experience working with individuals who have co-occurring mental health and substance use disorders.
  • Knowledge of community-based services, care coordination, and evidence-based behavioral health practices.
