Requirements
  • A high school diploma or GED is required.
  • At least 3 years of experience in the field of developmental disabilities is required, including at least 1 year of program supervision experience.
  • A bachelor's degree in Health and Human Services or a related field is preferred.
  • Ability to be on call to respond to emergencies, including incidents, safety hazards, and staffing issues.
Responsibilities
  • Coordinate, implement, and evaluate individual program plans and maintain records in compliance with OPWDD and state and federal regulations.
  • Hire, train, and supervise Direct Support Professionals in completing their job responsibilities.
  • Orient and evaluate residential staff, including progressive discipline and staff development, and secure records confidentially.
  • Implement facility and CFS policies, procedures, and programs in accordance with OPWDD regulations and guidelines.
  • Attend, participate in, and monitor or review team meetings, including Interdisciplinary, Individual Service Plan, and clinical meetings.
  • Attend and participate in agency standing committee meetings, training, and assigned activities outside the residence.
  • Ensure timely submission of required paperwork, including payroll, Res Hab documentation, and incident reports and investigations.
  • Ensure the residence physical plant is maintained and meets regulatory requirements.
  • Complete and ensure staff complete mandatory training, including SCIP, AMAP, CPR and First Aid, Corporate Compliance, and Choking Prevention.
  • Model and perform all aspects of individuals' program plans and ensure staff understand and implement each plan using a person-centered approach.
  • Facilitate communication with day programs and related work sites to support comprehensive case management.
  • Review documents in Therap, including checklists, t-logs, and medical information, daily, and address problems as they arise.
  • Conduct record reviews to ensure compliance and identify potential problems for resolution.
  • Communicate information to relevant parties, including the Assistant Director, Director, MSC, and BIS.
  • Develop and maintain relationships with individuals' family members and advocates and address concerns as they arise.
