- Current, active Texas Emergency Medical Technician-Basic certification.
- A valid Texas driver's license and valid auto insurance.

- Bilingual English/Spanish capability is strongly preferred given the patient population.

- Conduct scheduled and unscheduled in-person drive-by visits to potential home health patients who do not respond to phone contact attempts by the start-of-care operations team.
- Facilitate and support start-of-care processes by confirming patient presence, availability, and readiness for an initial skilled nursing or therapy evaluation.
- Serve as a liaison between the patient and the home health care team to reduce gaps in care and prevent unnecessary hospitalizations.
- Assist in verifying patient demographic and insurance information at the point of contact.
- Communicate findings in real time to the start-of-care Operations Team, Clinical Manager, and Sales Team member through appropriate communication channels.
- Perform basic assessments within the Emergency Medical Technician-Basic scope of practice to identify immediate safety concerns or urgent medical needs.
- Coordinate with the Clinical Manager when a patient is found in acute distress or requires potential emergency intervention.
- Maintain confidentiality and comply with all Health Insurance Portability and Accountability Act regulations.
- Work in the field across an assigned geographic territory, including early morning, evening, or weekend shifts as needed to reach patients at home.
- Prior experience in home health, community paramedicine, or emergency medical services.
- Bilingual English/Spanish capability given the patient population.
- Cardiopulmonary resuscitation/Basic Life Support certification from the American Heart Association.
