Requirements
  • A master's degree or higher in Social Work from a program registered by the Department of Education, with specific training dealing with adult patients.
  • A valid, unrestricted Licensed Clinical Social Worker or Master of Social Work license, or equivalent, in the state of employment or another U.S. state or territory as permitted by VA contractual requirements.
  • At least 2 years of post-master's direct counseling experience with adult patients, subject to individual VA contract requirements.
  • Mastery of advanced sociological, psychological, and mental health care interventions and issues.
  • Credentialing and continued good standing through the Veterans Health Administration.
  • Current Basic Life Support certification and Advanced Cardiovascular Life Support certification as required by the applicable Veterans Affairs medical center contract.
  • Proficiency in written and spoken English.
  • Strong computer skills, including electronic medical record experience.
  • Ability to work in a safety-sensitive position.
Responsibilities
  • Interview individual members to assess social and emotional capabilities.
  • Provide case management to Veterans and families throughout the continuum of care as needed.
  • Select programs appropriate to group goals, developmental level, needs, capacities, and interests.
  • Conduct individual counseling and therapy.
  • Conduct group therapy, including post-traumatic stress disorder, weight management, anger management, coping skills, grieving, and personal interaction groups.
  • Conduct marriage and family counseling as specified by the Department of Veterans Affairs.
  • Involve members in planning and assuming responsibility for activities.
  • Help members develop attitudes and social skills through group experiences to improve family relations and community responsibility.
  • Refer members to community resources and other organizations when indicated.
  • Address advance directives and organ donation in accordance with acceptable standards.
  • Enter patient information into VISTA and CPRS.
  • Complete all clinical reminders due at the time of each patient visit.
  • Assist with co-managed care collaboration with non-Veterans Affairs providers and facilities.
  • Provide patient and family health education focused on self-management, prevention, wellness, related health issues, and healthy lifestyle choices.
  • Maintain confidentiality of Veterans' data and information.
  • Comply with Veterans Affairs and company training requirements.
  • Fulfill compliance requirements for the Office of Inspector General, Joint Commission, Environment of Care oversight, laboratory compliance, and related areas.
  • Remain focused on achieving excellent clinical outcomes through specified Veterans Affairs guidelines.
  • Provide customer service to Veterans, their families, colleagues, and clinic visitors in person and by telephone.
  • Participate in clinic outreach events supporting enrollment initiatives.
  • Support patient enrollment and retention for appointments, follow-up visits, and nurse visits.
  • Support new clinical and operational initiatives.
  • Collaborate with the core Primary Care Team, including the primary care provider, registered nurse, and medical assistant, and with the expanded team involved in patient care planning.
