Requirements
  • A high school diploma or equivalent is required.
  • Completion of all required trainings within six months of hire is required.
  • Successful completion within six months of hire of the department-led or approved training program required by Medicaid regulations is required.
  • One year of full-time experience working with individuals receiving services for treatment of mental health or co-occurring disorders is required.
  • Formal supervisor documentation of the ability to function independently as a direct support professional and proficiency in providing direct support services is required.
  • Ability to provide caring and nurturing attention and care to individuals with disabilities, including medically fragile individuals and those with intense behavioral needs or high levels of risk, is required.
  • Ability to communicate with verbal and nonverbal individuals is required.
  • Teaching skills, patience, empathy, maturity, and judgment are required.
  • Ability to lift up to 100 pounds, with frequent lifting or carrying of objects weighing up to 50 pounds, is required.
  • Applicants must be at least 18 years old.
  • A valid driver's license, proof of automobile liability insurance, and a positive driving record are required.
Responsibilities
  • Teach, model, and coach activities of daily living, monitor clients' performance of those skills, and provide therapeutic feedback.
  • Teach psychoeducation classes on recovery, including medication side effects, activities of daily living, communication skills, and stress management strategies.
  • Plan and facilitate community-based activities and facilitate access to community resources.
  • Practice verbal de-escalation and, when authorized, other behavior management techniques to support a safe and therapeutic environment.
  • Integrate clients into the community and educate community members about people with disabilities and the organization's services and programming.
  • Collaborate with schools, landlords, families, transportation providers, employers, and other community systems on behalf of individuals served.
  • Provide transportation using an organization van or personal vehicle, follow safety practices, and teach and facilitate use of public transportation systems.
  • Provide personal care as needed, including hygiene, toileting, laundry, meal preparation, and safe eating techniques.
  • Administer first aid and cardiopulmonary resuscitation as needed.
  • Supervise self-administered medication or administer medication according to the applicable clinical program's protocols.
  • Identify clients' food and medication allergies and exercise reasonable care to avoid allergen exposure.
  • Communicate information relevant to care, treatment, and services with staff and authorized non-staff providers.
  • Take responsibility for specific program assignments when needed.
  • Provide limited on-the-job training to Community Support Providers by answering questions, providing shadowing and observation opportunities, and explaining site programming.
  • Document services and care provided by completing required forms in accordance with organizational and regulatory standards.
  • Maintain confidentiality of protected health information in accordance with organizational procedures and state and federal law.
  • Attend and participate in required staff meetings, service planning meetings, and trainings.
  • Maintain and enhance skills for working with clients.
