Requirements
  • A minimum of three years of clinical nursing experience within the last seven years.
  • A minimum of two years of inpatient case or care management experience within the last five years.
  • A baccalaureate degree in Nursing from a program accredited by the Commission on Collegiate Nursing Education, Accreditation Commission for Education in Nursing, or Commission for Nursing Education Accreditation, completed 30 days before the start date; or an associate degree in Nursing from an accredited program completed 30 days before the start date.
  • Associate degree nurses must enroll in a Bachelor of Science in Nursing program within three years of hire and complete it within five years of hire.
  • A current, unencumbered Oregon State Registered Nurse license is required.
  • Basic Life Support certification from the American Heart Association is required; new hires will complete it during orientation.
  • Ability to perform the essential functions of the position with or without accommodation.
  • RN case managers must comply with the regulatory requirements of DNV, Medicaid, Medicare, and the Oregon Nurse Practice Act.
Responsibilities
  • Assess patients on admission for available family support, insurance resources, and potential discharge planning needs.
  • Engage in daily multidisciplinary rounds to develop and implement patient discharge plans with input from other disciplines.
  • Coordinate post-acute and discharge needs in a timely and cost-efficient manner.
  • Use established criteria to determine the appropriateness of continued hospital stay.
  • Work with payors to obtain authorizations for ongoing hospitalization and identified post-acute needs.
  • Collaborate with patients and families, multidisciplinary team members, community resources and agencies, and payors to ensure that patient progress and level of care are appropriately determined.
  • Practice utilization and resource management based on individual patient needs.
  • Lead multidisciplinary and family care conferences involving complex care situations.
  • Contribute to reducing length of stay and readmission rates.
  • For emergency department assignments, conduct comprehensive patient and family assessments and admission clinical reviews upon emergency department admission.
  • Identify high-risk patients and their needs early.
  • Perform care coordination and discharge planning for emergency department and observation-unit patients.
  • Assess patients' potential for discharge from the emergency department with community care coordination.
  • Round with advanced practice providers at designated times twice daily.
  • Assist the transfer center with coordinating appropriate patient transfers out of the emergency department.
  • Educate providers about documentation requirements supporting medical necessity determinations.
  • Deliver Medicare notifications, including Condition Code 44 notices, observation notices, and Medicare Important Messages.
  • Document according to departmental policy.
  • Lead efforts to ensure compliance with Centers for Medicare & Medicaid Services and other insurance regulations related to care management.
  • Provide evening-shift nurse case management support to patients, families, and staff in inpatient and emergency department settings seven days per week.
  • Receive referrals and handoffs from day-shift case managers.
  • Complete information handoffs to day-shift case managers.
  • Troubleshoot and resolve discharge delays or barriers carried over into the evening shift.
Desired Qualifications
  • Case management certification such as ACMA, RN-BC, or CMSA.
  • Hospital-based case management experience.
  • Proficiency in managing difficult and sensitive family and provider care conferences involving adjustment to new levels of function, end-of-life discussions, and imbalances in social, spiritual, medical, and financial resources.
  • Progressive leadership experience within a nursing unit, such as charge nurse experience or participation in a UBNPC, shared governance council, or similar nursing leadership body.
