About the job

Requirements
  • A college diploma (DEC) in social work techniques or delinquency intervention techniques.
  • Be a member in good standing of the Quebec Order of Social Workers and Marriage and Family Therapists (OTSTCFQ).
  • Have OCCI training.
  • Be willing to work in a northern or remote community.
Responsibilities
  • Assess residents' psychosocial needs.
  • Collaborate with the interdisciplinary team to plan and follow up on intervention plans.
  • Support residents and their families as they adapt to life in a long-term care facility (CHSLD).
  • Coordinate community or external services as needed.
  • Maintain clinical documentation in accordance with professional standards.
Desired Qualifications
  • Experience working with older clients in an institutional setting.
