Requirements
  • Graduate from an accredited Practical Nursing program.
  • Hold a current LPN/LVN license in the state of employment.
  • Have a minimum of two years of LPN experience in hospice, oncology, long-term care, or home health.
  • Hold CPR certification.
  • Have a valid driver's license and current automobile insurance.
  • Understand hospice care principles and the unique needs of terminally ill patients and their families.
  • Demonstrate patient assessment and clinical skills.
  • Have communication and documentation abilities.
  • Be familiar with regulatory guidelines including Medicare, Medicaid, CMS, and JCAHO standards.
  • Be able to work independently and as part of an interdisciplinary team.
Responsibilities
  • Provide direct nursing care services to hospice patients, supporting patients and their families.
  • Assist the interdisciplinary team in assessing patient and family needs and contribute to individualized care plans.
  • Report changes in patient conditions promptly to the Registered Nurse and care team.
  • Deliver care aligned with each patient's plan of care.
  • Support families during difficult times.
  • Provide skilled bedside nursing care under the supervision of a Registered Nurse, following established nursing protocols and the Nurse Practice Act.
  • Monitor patients and document changes in condition, reporting as necessary.
  • Administer medications and treatments according to the plan of care.
  • Participate in infection control practices and standard precautions.
  • Support bereavement activities and closure contacts with families as assigned.
  • Participate in on-call duties and respond to patient needs during scheduled visits or emergencies.
  • Assist with death pronouncements when permitted by state law and under Registered Nurse supervision.
  • Collaborate with hospice aides and other team members to ensure comprehensive care delivery.
Desired Qualifications
  • Three to five years of LPN experience is preferred.
