About the job

Requirements
  • The candidate must have 1–4 years of software engineering experience.
  • The candidate must have strong full-stack execution with a backend bias and clear evidence of having shipped real software.
  • The candidate must be able to communicate clearly and demonstrate product judgment, including talking to customers, asking clear questions, and turning ambiguity into implementation across the engineering-product-design cycle.
  • The candidate must demonstrate high ownership and urgency, finding the shortest path from customer pain to working software in messy, real-world environments.
Responsibilities
  • Sit with providers, payers, and operations teams to understand how authorizations, claims, denials, appeals, and payment workflows operate.
  • Translate workflow observations into product changes that ship quickly and safely.
  • Debug live customer issues across charts, claims, payer portals, electronic health record and practice management systems, policy rules, and operational queues.
  • Identify workflow gaps that may not appear to be bugs but still damage the customer experience.
  • Work directly with the chief executive officer and engineering team from pilot through customer close and post-close expansion.
  • Travel to customers when it materially improves product understanding and deployment quality.
Desired Qualifications
  • Healthcare experience is helpful but not required.

About the company

Backbone Systems builds an AI-powered platform that acts as a verification layer for healthcare payments. It analyzes patient charts and clinical records against live payer rules and contracts to validate care before service and to check codes before billing. The system automatically flags coding errors, detects bundling or carve-out opportunities, and issues denial risk alerts to improve first-pass payment rates and reduce rework for revenue cycle teams. The company differentiates itself with a paid-or-credit guarantee on approved claims, offering protection against clawbacks and refunds if coverage isn’t secured. Its goal is to create intelligent financial rails that enable faster, more reliable payments between healthcare providers and payers. Backbone operates on a B2B SaaS model targeting providers’ revenue cycle teams, and has notable investors such as Andreessen Horowitz, Lightspeed, and Hanabi.

What believers are saying

  • Backbone posted Founding Engineer roles in May and June 2026, signaling active product buildout.
  • Its website, updated Aug. 5, 2026, highlights referral, longitudinal-chart, and medical-necessity automation.
  • The CMS prior-authorization push strengthens demand for faster, auditable provider-side verification tools.

What critics are saying

  • Anterior raised $40 million on Feb. 12, 2026, intensifying prior-authorization competition.
  • Cohere Health already sits inside payer workflows and targets January 1, 2027 mandates.
  • Claim guarantees can explode losses if payer denials, coding disputes, or clawbacks outrun reserves.

What makes Backbone Systems unique

  • Backbone’s Aug. 5, 2026 site promises pre-bill chart review, not denial triage.
  • Its paid-or-credit guarantee shifts reimbursement risk onto Backbone, unlike typical RCM software.
  • Manan Shah’s Stanford and Kumo background suits infrastructure-heavy healthcare AI systems.
