Requirements
  • Associate or bachelor’s degree in public health, healthcare administration, or a related field required.
  • 3+ years’ experience in healthcare or benefits assistance, particularly in-patient coordination or administrative roles, research, public health, case management or customer service required.
Responsibilities
  • Manage and coordinate referrals to community and government services to ensure comprehensive support for patients with complex Health-Related Social Needs (HRSN).
  • Conduct in-depth patient assessments; identify best-fit resources for patients that involves incorporation of patient-level needs, resource eligibility, and insurance benefits.
  • Implement optimal referrals for various services; troubleshoot challenges that arise in patients receiving needed services.
  • Serve as a team leader to coordinate management of patient caseloads and ensure loop closure for all open cases.
  • Collaborate with care team regarding HRSN results, interventions, and update the team on referral outcomes and patient satisfaction with resources.
  • Conduct HRSN assessments and other critical screenings onsite in various clinical and community settings for patients and community members.
  • Document screening results accurately and follow workflows to communicate positive screenings to relevant staff.
  • Educate patients on available resources, guide them on independent access, and assist in completing necessary paperwork for program approval.
  • Conduct follow-up assessments and provide ongoing support to ensure individuals receive services related to referrals (loop closure).
  • Schedule appointments for patients as needed with a variety of services.
  • Engage regularly with community-based organizations (CBOs) to ensure up-to-date information regarding capacity, client eligibility, referral processes, etc. and to troubleshoot specific patient referrals and general workflows.
