Requirements
  • Candidates must hold both of the following credentials: Board Certification in Internal Medicine from the American Board of Internal Medicine or Family Medicine from the American Board of Family Medicine, and subspecialty certification in Geriatric Medicine from the American Board of Internal Medicine or a Certificate of Added Qualifications in Geriatric Medicine from the American Board of Family Medicine.
  • Have at least 5 years of clinical practice experience and an active medical license in at least one state without restrictions.
  • Have experience working with interdisciplinary care teams.
  • Have experience within a high-growth startup or digital health environment.
  • Be able to communicate effectively in a remote-first environment and use asynchronous tools such as Slack and Notion.
Responsibilities
  • Assess, comprehensively diagnose, and provide longitudinal care for patients living with dementia.
  • Evaluate and manage a panel of patients with cognitive disorders and their caregivers.
  • Lead members of the interdisciplinary team, including Neurologists, Geriatricians, Care Navigators, Registered Nurses, Nurse Practitioners, Licensed Clinical Social Workers, and Pharmacists.
  • Lead, supervise, and mentor multidisciplinary care-team members to deliver high-value patient care.
  • Stay current with research and advances in dementia care and translate this knowledge to the care team, including guidance on social determinants of health, fall-risk assessment and intervention, and high-risk medications.
  • Collaborate with and provide oversight of the interdisciplinary team by staffing questions requiring geriatric expertise.
  • Identify improvements to the care model and technical platform.
  • Contribute to the program evaluation framework by identifying metrics for dyad outcomes, clinician satisfaction, and care-model improvement.
  • Spearhead population-health initiatives and refine care models using real-world evidence and emerging clinical best practices.
  • Collaborate with the Medical Director and Clinical Operations to deliver clinical key performance indicators, including quality targets, utilization metrics, and patient and provider satisfaction.
  • Develop, implement, and maintain regular educational opportunities for clinical providers, including clinical case conferences and guest lectures, with the Medical Director.
  • Translate the organization's care philosophy into functional clinical requirements for internal tools and patient-facing interfaces.
  • Provide clinical validation for new features and ensure technical iterations meet medical standards.
  • Work with the Medical Director on hiring and training new providers as the organization expands into new markets.
Desired Qualifications
  • An active, unrestricted medical license in Washington, Oregon, Idaho, or Ohio.
  • Completion of an ACGME-accredited Geriatric Medicine fellowship and specialized training in age-friendly care.
