Requirements
At least 2 years of clinical experience is required; the posting also states 3 years of clinical-setting experience.
Be a registered nurse with an active Tennessee license, or hold a license in the state of residence if it participates in the Nurse Licensure Compact Act; alternatively, hold a Master’s in Social Work with an active unrestricted license (LCSW, LMSW, or LAPSW).
Live in Shelby County, Tennessee.
Be available to work 8:00 a.m. to 5:00 p.m. Eastern Time; there is no on-call requirement. With management approval, a compressed work week may be available after one year.
Have a valid driver’s license. Employees routinely driving a company, personal, or rental vehicle for business must comply with the Driver Safety Program, including applicable auto insurance requirements and maintaining an acceptable motor vehicle record.
Complete a TB skin test if working in the field; other immunizations and/or associated medical tests may be required.
Complete a digital literacy assessment.
Have basic personal computer skills, including Microsoft Office and email.
Have effective time-management skills.
Have strong oral and written communication skills.
Have strong interpersonal and organizational skills.
Remain in the role for 24 months before becoming eligible to apply for other internal positions.
Responsibilities
Partner with members and families to identify needed supports and direct services that meet personal goals for health, employment, and independent or community living.
Collaborate with clinical and social support colleagues to meet each member’s physical and behavioral health needs and long-term service needs.
Conduct thorough, objective, face-to-face visits and assess each member’s situation to determine current status and needs, including physical, behavioral, functional, psychosocial, financial, employment, and independent-living expectations.
Use criteria to authorize appropriate home- and community-based services, and confirm that services are being provided and members’ needs are being met.
Travel to members’ homes for visits and manage requests and demands from internal and external stakeholders.

Employees who are required to operate either a BCBST-owned vehicle or a personal or rental vehicle for company business on a routine basis* will be automatically enrolled into the BCBST Driver Safety Program. This includes, maintaining a valid driver’s license, auto insurance compliance with minimum liability requirements; as defined in the “Use of Non BCBST-Owned Vehicle” Policy (for employees driving personal or rental vehicles only); and maintaining an acceptable motor vehicle record (MVR).
