Requirements
  • A specialized diploma from an accredited school of nursing is required.
  • Three years of experience as a registered nurse in an acute care setting is required.
  • Effective communication skills are required, including the ability to transmit, receive, and accurately interpret ideas, information, and needs through appropriate communication behaviors.
  • Knowledge of patient- and family-centered practice is required, including conveying respect and collaborating with each patient, family, or designated care partner.
  • Cultural awareness is required, including the ability to convey respect and connect effectively with people from diverse backgrounds.
  • Knowledge of care coordination is required, including knowledge of medical, social, economic, and other services provided by departments or facilities and the ability to integrate multidisciplinary treatments for optimal patient care.
  • Knowledge of patient safety is required, including the ability to identify risks in healthcare environments and take steps to reduce them.
  • Knowledge of medication administration and prescriptions is required, including applying this knowledge to administering medications and prescribing drugs for continuing treatment outside the hospital.
  • Knowledge of discharge planning is required, including considering patients' post-discharge needs and arranging follow-up care.
  • Knowledge of assessment and diagnosis is required, including using patient-assessment tools and techniques, making nursing diagnoses or exercising clinical judgment, analyzing reasons behind symptoms, and generating resolutions based on available drugs, facilities, and techniques.
  • Knowledge of health information documentation is required, including recording medical histories, symptoms, vital signs, medications, diagnoses, treatment plans, procedures, tests, and medical results in written or electronic form.
  • A registered nurse license in Pennsylvania is required.
  • An active registered nurse license in the state of practice is required.
Responsibilities
  • Assist in leading the daily operations of a specific unit.
  • Act as a resource, lead, and clinical role model for the patient care team.
  • Advance and oversee patient care standards for the designated shift.
  • Facilitate and coordinate patient care delivery and flow through the continuum of care.
  • Support nursing professional development, Magnet, and Shared Governance through participation in and mentoring of colleagues on councils and the Clinical Ladder program.
  • Interact with co-workers, visitors, and staff in accordance with Jefferson's values.
  • Recognize patients and their families or designated care partners when providing safe, compassionate, and coordinated care based on personal preferences, priorities, needs, and cultural beliefs and values.
  • Communicate with and support interprofessional teams and services, patients, families, and designated care partners.
  • Serve as a clinical resource regarding policies and procedures, patient standards, performance improvement, and risk-management issues.
  • Collaborate with medical staff to facilitate consistent interdisciplinary plans of care and ensure quality patient care.
  • Assess patient needs and make care recommendations based on physical, psychosocial, cultural, age, and functional needs.
  • Facilitate timely patient throughput optimization for admissions, transfers, and discharge on the designated unit.
  • Participate in plan-of-care rounds according to service and unit protocols when applicable.
  • Provide timely Smart Square schedules and on-call schedules when applicable.
  • Follow up on and effectively resolve customer and physician issues and complaints.
  • Monitor staff schedules to ensure adequate staffing and maintain quality patient care.
  • Authorize overtime or scheduling changes to fill shifts when necessary.
  • Complete payroll in collaboration with the Nurse Manager.
  • Provide volumes and adjustments in Smart Square to reflect variances in staffing needs.
  • Collaborate with the Nurse Manager to evaluate, maintain, and improve patient care team member performance.
  • Assist in implementing changes in clinical practice and evaluate the quality and effectiveness of nursing practice to ensure compliance with organizational, department, and unit-specific goals.
  • Provide patient care support when necessary.
Desired Qualifications
  • A Bachelor of Science in Nursing or completion of a Bachelor of Science in Nursing, or an appropriate advanced degree, within three years of hire is strongly preferred.
  • Certification in the specialty is preferred; when certification is required on a specific unit, it must be obtained within six months of hire.
