Requirements
  • Thorough knowledge of social work principles and methods.
  • Knowledge of individuals and group behavior and ways of working effectively with adults, children, and families who have social, economic, emotional, or health problems.
  • Ability to exercise independent professional judgment in complex situations.
  • Ability to establish and maintain effective working relationships with clients and families, staff members, outside agencies and institutions, and the general public.
  • Ability to communicate effectively both orally and in writing.
  • A Master’s degree in Social Work is required.
Responsibilities
  • Assess the psychosocial status of clients and the social and emotional factors related to their illness, need for care, and response to treatment.
  • Provide direct social casework services to clients and families, including assisting with goal setting, counseling, planning, and interventions.
  • Advise agency personnel, clients, and families about appropriate community resources, eligibility criteria, and regulations.
  • Participate in the development and periodic renewal of the client’s plan of care.
  • Observe and report to the physician the client’s response to treatment and changes in the client’s condition.
  • Maintain verbal and/or written communication with other personnel involved in the client’s care.
  • Maintain appropriate and timely client care records, including observations, treatments, client response to treatment, and changes in the client’s condition.
  • Attend and participate in appropriate client care conferences, staff meetings, and agency committee meetings.
  • Assist patients and families in coping with problems resulting from severe or long-term illness.
  • Assess, diagnose, and treat patients’ mental and social conditions, counsel individuals and/or families, and update case records.

Brighton Hospice is a hospice provider supporting people with life-limiting illnesses and their families. Interdisciplinary teams coordinate nursing, medical, social-work, spiritual, aide, and bereavement support. Care is delivered in homes and other residential settings with an emphasis on comfort, dignity, and quality of life. Brighton serves patients who have elected comfort-focused end-of-life care and works with caregivers and referring clinicians through an interdisciplinary model combining symptom management, psychosocial support, spiritual care, and bereavement services. Services are coordinated around an individualized plan of care and changing symptoms.
