Responsibilities
  • Conduct thorough patient assessments to ensure personalized, comprehensive care.
  • Guide and educate patients and their families on health maintenance and disease prevention.
  • Assist with activities of daily living as needed while maintaining patients’ dignity and respect.
  • Coordinate schedules directly with patients in accordance with their plan of care, approved hours, and preferences.
  • Monitor and promptly report changes in patient symptoms or behavior.
  • Administer medications and treatments as prescribed by the treating physician.
  • Communicate with case managers, collaborating physicians, and specialists to coordinate patient care.
  • Maintain accurate patient records in the electronic medical record system.
  • Provide advice and emotional support to patients and their family members.
  • Prepare clear clinical notes and charting.
