  • A Bachelor of Science in Nursing degree is required upon hire for new hires.
  • Three years of experience in an acute care hospital setting is required.
  • A Registered Nurse license in good standing with the State of Michigan is required.
  • Case Management Certification is preferred.

Responsibilities
  • Ensure appropriate care based on patient needs and hospital capabilities throughout admission, care delivery, and discharge.
  • Perform early assessment and identify patients at risk for post-hospitalization care and services.
  • Assess patients and families, review relevant health records, and evaluate psychosocial care needs.
  • Develop, implement, coordinate, communicate, and revise patient transition and care plans with the care coordination team.
  • Monitor patient progress, document according to procedure, maintain ongoing contact, and round daily on assigned patients.
  • Identify readmissions, determine reasons and needed interventions, and communicate plans to the multidisciplinary team.
  • Drive multidisciplinary team rounds and coordinate smooth transitions between services.
  • Document transitional planning and enact plans that move patients along the care continuum.
  • Identify and allocate post-discharge services and resources, including medical social work, pastoral care, rehabilitation, long-term care, home health care, and community resources.
  • Make appropriate referrals and act as a liaison among patients, physicians, ancillary services, community services, and providers.
  • Build and maintain collegial relationships with physicians, nursing teams, and leaders.
  • Coordinate care using Pathways or Plans of Care and contribute to their ongoing development and revision.
  • Participate in assigned groups and committees.
  • Manage discharge plans and coordinate services and resources to support quality, clinical, financial, and length-of-stay outcomes.
  • Evaluate case-management outcomes, including length of stay, readmissions, patient satisfaction, and financial variances, and report pertinent variances.
  • Track and trend outlier length-of-stay data to reduce outlier stays.
  • Coordinate and oversee hospital operations during House Manager assignments and provide clinical and administrative direction when Unit Leaders are absent.
  • Resolve crises and conflicts, investigate problems and complaints, assess safety concerns, and initiate appropriate interventions and follow-up.
  • Coordinate admissions, transfers, and patient placement; monitor hospital capacity and staff placement.
  • Conduct hospital rounds, maintain utilization review, monitor infection-control data, and lead weekend and holiday Safety Check-In meetings.
  • Handle after-hours Employee Incident Hot Line calls, perform initial triage, document incidents, and coordinate exposure-related treatment with the emergency-room physician.
  • Work with multidisciplinary teams and providers to create standard patient care plans and review care plans or behavioral contracts with patients.
  • Assess uninsured and underinsured patients for eligibility for federal, state, and community programs, assist with funding paperwork, and collaborate with Patient Financial Counselors.
  • Maintain a network of community contacts and participate in community meetings to identify resources for patient needs.
Desired Qualifications
  • A master's degree is preferred.
