- Hold a current Licensed Practical Nurse license in Virginia.
- Have at least one year of nursing experience.
- Maintain current CPR certification.
- Be able to function with minimal supervision.
- Be able to manage multiple projects and changing priorities.
- Be able to employ critical thinking skills.
- Accept client assignments from the Registered Nurse Case Manager.
- Provide nursing care using home assessments, nursing diagnoses, and physician-approved care plans.
- Perform treatments ordered by the physician and delegated by the Registered Nurse Case Manager.
- Observe signs and symptoms and report them to the Registered Nurse Case Manager.
- Note changes in clients' physical or emotional status.
- Promote client and caregiver independence in self-care.
- Use electronic documentation for client care and comply with agency policies regarding timely submission of documentation and data synchronization.
- Follow agency policies regarding universal precautions and safety guidelines.
- Fulfill other assignments as requested by the Registered Nurse Case Manager, Vice President of Clinical Services, or Director of Professional Services.
