Requirements
  • A high school diploma or GED and three years of patient care or community resource experience are required.
  • A valid driver's license and reliable transportation are required.
  • The ability to meet the stated sitting, standing, walking, hand-use, touch, hearing, and vision requirements is required.
  • The Community Health Worker course must be completed within one year of hire.
  • Confidentiality, attendance, dress code, and network and departmental policy requirements must be followed.
Responsibilities
  • Complete assigned care plan tasks to address social determinants of health barriers and work toward care plan goals.
  • Develop and maintain relationships with community agencies and resources.
  • Establish trusting relationships with patients and families through home visits, community visits, and telephone outreach.
  • Screen for home safety and health concerns.
  • Provide informal health coaching on self-management tools related to chronic illnesses.
  • Assist with telehealth visits, technology access, SLUHN.org and MyChart access, and preparation of patient questions for the registered nurse care manager.
  • Assist patients and the registered nurse care manager with medication reviews during home or community visits.
  • Use motivational interviewing techniques and scripting during outreach to patients and caregivers.
  • Attend provider and community appointments with patients as directed by the manager.
  • Provide referrals to community agencies using Findhelp when appropriate.
  • Maintain timely, accurate, complete, and consistent documentation in the electronic medical record.
  • Assist patients with applications and registration forms.
  • Maintain expertise in telehealth procedures.
  • Participate in staff meetings, network or departmental committees, special projects, rounding, site visits, and on-site meetings or education as required.
  • Screen the need for referrals and determine eligibility for services according to government and local program guidelines.
  • Support network and departmental goals and objectives.
  • Manage time appropriately for home visits and documentation.
  • Maintain knowledge of current care management programs available to patients and families.
  • Participate in precepting new employees as assigned.
  • Participate in peer educational presentations.
  • Use organizational resources effectively and efficiently.
  • Perform other related duties as assigned.
