Requirements
  • Must possess a valid current or temporary Alaska Licensed Practical Nurse license.
  • Requires the knowledge and skills normally demonstrated by an Associate's degree in nursing.
  • Basic Life Support certification is required.
  • May require Advanced Cardiovascular Life Support, Pediatric Advanced Life Support, or additional certification and/or continuing education based on area of practice.
  • Requires creative thinking, prioritizing, and flexible problem-solving skills.
  • Must be able to communicate clearly in both verbal and written forms.
Responsibilities
  • Observe, compile, and report the patient's physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs.
  • Contribute to the plan of care under the direction of a registered nurse, including the discharge plan, using assessment data and patient, family, and health team input.
  • Plan care in collaboration with members of the multidisciplinary team and review the plan of care to reflect changing patient needs.
  • Implement delegated interventions identified in the care plan and medical orders within the scope of practice.
  • Provide care based on physician orders and the nursing plan of care in compliance with policies, procedures, standards of care, and regulatory requirements.
  • Delegate and supervise the provision of care to non-licensed personnel when permitted by state scope of practice.
  • Communicate the status of assigned patients accurately and completely to other caregivers to promote continuity of care.
  • Report patient conditions, including signs and symptoms that may indicate a change in condition, to the registered nurse and/or physician.
  • Observe and report patient progress toward goals and expected outcomes in collaboration with health care team members.
  • Observe and report the patient's response to and the effectiveness of patient teaching.
  • Document observations, interventions, data, and patient responses in the patient record.
  • Record physician orders accurately and in a timely manner.
  • Plan, organize, develop, and coordinate documentation and patient assessment data as required by federal regulation when assigned.
  • Maintain, update, and revise required documentation according to patient condition changes and federal or state guidelines.
  • Communicate and coordinate with the interdisciplinary team regarding appropriate functional or care level and transitions to another level.
  • Perform audits related to reimbursement documentation and the quality management process, and present summary information for management review.
  • Coordinate the admission process, including new resident evaluations, when assigned.
  • Provide verbal and written communication to residents, families, and referral sources regarding the admission process.
  • Facilitate problem-solving with residents, families, referral sources, insurance companies, and clinical staff.
  • Perform all functions according to established policies, procedures, regulatory and accreditation requirements, and applicable professional standards.
