Requirements
  • A minimum Bachelor's degree is required.
  • Experience working with people living with AIDS, homeless people, or substance users is required.
  • Effective written and oral communication skills are required.
Responsibilities
  • Provide direct, field-based services to enrolled consumers.
  • Conduct outreach at SRO field sites and community organizations to recruit and screen potentially eligible SRO residents.
  • Conduct presentations and meetings with medical providers and community organizations to facilitate referrals.
  • Conduct home visits to connect or re-engage consumers with medical providers.
  • Identify, research, and report areas for potential consumers.
  • Follow and update the outreach referral log and weekly in-home visit schedules.
  • Provide critical feedback to the Care Coordination Redesign team based on field observations.
  • Facilitate groups for Care Coordination Redesign consumers as needed.
  • Participate in monthly team meetings, supervision, and required training.
  • Participate in Services for the Underserved quality assurance and quality improvement activities.
  • Record all service provision on Field Logs or Case Finding forms daily for submission to the Data Entry Specialist.
  • Generate monthly outreach reports.
  • Work at least 80% of the time in the community.
  • Perform other duties as directed.
Desired Qualifications
  • Bilingual Spanish ability is encouraged.
  • Experience working within an outreach model or in case management is preferred.
  • Basic computer skills, including Microsoft Office and Windows, are preferred.
