Requirements
  • One to two years of experience or training in a related function.
  • A high school diploma or equivalent is required.
  • A valid driver's license, adequate vehicle insurance when using a personal vehicle, and a safe driving record are required.
  • Applicants must be at least 21 years of age, have maintained a valid United States driver's license for at least six months, and pass a driver's screening background check.
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, lift at least 25 pounds using proper lifting techniques or a two-person lift, and maneuver through narrow spaces or hallways.
  • Ability to operate a computer and office equipment such as a calculator, copier, and printer.
  • Ability to assist clients with activities of daily living and communicate with clients, staff, and external representatives.
  • Required vision abilities include close, distance, color, peripheral, and depth perception, plus the ability to adjust focus.
  • Cardiopulmonary resuscitation certification is required within two weeks of hire.
  • First Aid certification is required within two weeks of hire.
  • Medication Administration Program certification is required within 150 days of hire.
  • Safety Care certification is required within 90 days of hire.
  • Positive Behavioral Supports Tier II certification is required before hire.
  • Nonviolent Physical Crisis Intervention certification is required.
  • Ability to use advocacy, empowerment, self-advocacy, family partnership, social inclusion, participatory planning, personal management, household management, and crisis intervention techniques.
  • Knowledge and proven ability to implement Positive Behavioral Supports treatment programs.
  • Ability to work collaboratively and independently in a professional and confidential capacity, communicate effectively in oral and written formats, use different communication styles, solve problems, make decisions in stressful situations, and adapt to new skills.
Responsibilities
  • Assist individuals in becoming integrated into their communities and developing and maintaining friendships.
  • Assist individuals in accessing community-based support and resources.
  • Plan recreational and social activities that promote social inclusion and emotional well-being.
  • Empower individuals to make choices, self-advocate, and participate in decisions about their supports and lives.
  • Support access to assistive technology and adaptive equipment to augment communication, community access, and independence.
  • Support individuals in securing, maintaining, and growing work, volunteer, and other community engagement opportunities.
  • Encourage individuals and families to maintain strong partnerships with staff and other interested parties.
  • Provide administrative support to the Program Director, including scheduling shift assignments, training new staff, and helping oversee clinical paperwork.
  • Function as designated lead staff for shift management and clinical programming.
  • Assist in developing, implementing, and documenting treatment, including Individual Support Plans, Person-Centered Support Plans, and clinical programs.
  • Provide training and support covering rehabilitative and behavioral interventions, fire and safety regulations, appointment scheduling, money management, activities of daily living, grocery shopping, meal planning and cooking, household chores, and use of community resources.
  • Perform hands-on program tasks to implement support plans.
  • Provide transportation as necessary.
  • Assess individuals' physical and emotional status and report changes to a supervisor.
  • Monitor individual funds and maintain individual records, behavior and academic data, progress notes, incident documentation, program documentation, routine activity records, communication logs, community membership logs, and activity schedules.
  • Procure, store, administer, document, and dispose of medication in accordance with Medication Administration Program requirements, funding-source regulations, and company policy.
  • Communicate with program nurses, case managers, Visiting Nurses Association representatives, and other interested parties.
  • Participate in mandated training and staff meetings and assist with new-staff orientation.
  • Ensure compliance with internal and external regulatory policies, procedures, and standards.
  • Ensure home safety and maintenance, including housekeeping, snow shoveling, and trash removal.
  • Provide shift coverage in other homes or programs as assigned.
  • Maintain required staff-to-individual ratios and assist the Program Director in securing shift coverage.
  • Function as a human rights advocate, provide physical intervention or First Aid during behavioral or medical emergencies, and serve as a reporter to the Disabled Persons Protection Commission.
  • Perform other related duties as required.
Desired Qualifications
  • A Bachelor's degree is preferred.
  • Certified Brain Injury Specialist certification may be obtained.
