Requirements
  • Current Registered Nurse (RN) licensure in good standing in the state or states of practice.
  • Ability to collaborate daily with nurses, social workers, chaplains, volunteers, and certified nursing assistant leadership in a team environment.
  • Ability to work independently on a day-to-day basis with phone, email, and written communication; meetings with healthcare professionals; and an understanding of hospice rules and regulations.
  • Time management skills to manage visits, documentation, meetings, scheduled visits, required meetings, urgent needs, and multiple competing concerns through prioritization and triage.
  • Ability to learn an electronic medical records system.
  • Willingness to drive with reliable transportation, a valid driver's license, and auto insurance to travel across the service area.
Responsibilities
  • Provide care and services for a group of patients by visiting them where they reside, including private homes, skilled nursing facilities, and assisted living facilities.
  • Contribute as a member of the interdisciplinary team by providing expertise regarding psychosocial assessments, interventions, and community resources.
  • Work in a Case Management, Admissions, or Resource role.
  • Work closely with patients on a one-to-one level while receiving support from the patient care team.

Brighton Hospice is a hospice provider supporting people with life-limiting illnesses and their families. Interdisciplinary teams coordinate nursing, medical, social-work, spiritual, aide, and bereavement support. Care is delivered in homes and other residential settings with an emphasis on comfort, dignity, and quality of life. Brighton serves patients who have elected comfort-focused end-of-life care and works with caregivers and referring clinicians through an interdisciplinary model combining symptom management, psychosocial support, spiritual care, and bereavement services. Services are coordinated around an individualized plan of care and changing symptoms.
