Requirements
  • A minimum of three years of pediatric nursing experience is required.
  • Graduation from an approved school of professional nursing is required.
  • A Bachelor of Science in Nursing degree is required.
  • Current Illinois professional nursing licensure is required.
  • An Advanced Practice Nurse license is required if appropriate.
  • Current certification in cardiopulmonary resuscitation issued by the American Heart Association or American Red Cross is required.
  • Demonstrated knowledge and expertise in pediatrics, specialty areas, and family education are required.
  • Active membership in appropriate professional organizations is required.
Responsibilities
  • Coordinate care for pediatric patients throughout the healthcare continuum and evaluate patients before admission when needed.
  • Collaborate with families and healthcare teams to develop comprehensive plans of care based on physiological, psychosocial, and developmental needs.
  • Create individualized patient and family education plans and ensure that education is completed appropriately.
  • Evaluate and modify plans of care, provide follow-up between hospitalizations or clinic visits, and monitor patient progress toward established goals.
  • Act as a hospital liaison for families, external agencies, and individuals providing services to specific families.
  • Assist with plans of treatment, prescriptions, school forms, and similar documents with medical-provider approval.
  • Consult on patient-care issues, serve as a resource to nursing staff and community members, and perform trained clinical assessments or procedures when competent.
  • Develop and maintain data systems to track patient outcomes.
  • Provide clinical leadership through professional organizations, hospital committees, presentations, and publications.
  • Participate in or lead quality-improvement activities.
  • Perform other duties as assigned.
  • Make facility site visits and home visits when required by the patient population.
Desired Qualifications
  • A Master of Science in Nursing degree is preferred.
  • Experience in case management or a clinical specialty is desirable.
  • Certification in pediatric nursing, case management, or a specialty is preferred.
  • Ability to speak Spanish is desirable.
