- Outreach and engage individuals at risk of overdose.
- Connect clients to harm reduction services, including naloxone training and kits and fentanyl test strips.
- Complete or document clinical assessments, treatment and care planning activities, and continuing care management per ASAM guidelines.
- Use SI PPS systems to track outreach and engagement activity.
- Conduct Health Related Social Needs assessments using SI PPS technology and make referrals to treatment and recovery services.
- Ensure each participant signs a 42 CFR-compliant universal consent, documented electronically using SI PPS systems, so information can be shared between participating agencies and SI PPS.
- Track and report outreach, engagement, and outcomes to SI PPS on a quarterly basis.
- Participate in biweekly case conferences to review outreach and engagement activities.
