Requirements
  • A high school diploma is required.
  • A Certified Recovery Peer Advocate (CRPA) certification is required.
Responsibilities
  • Outreach and engage individuals at risk of overdose.
  • Connect clients to harm reduction services, including naloxone training and kits and fentanyl test strips.
  • Complete or document clinical assessments, treatment and care planning activities, and continuing care management per ASAM guidelines.
  • Use SI PPS systems to track outreach and engagement activity.
  • Conduct Health Related Social Needs assessments using SI PPS technology and make referrals to treatment and recovery services.
  • Ensure each participant signs a 42 CFR-compliant universal consent, documented electronically using SI PPS systems, so information can be shared between participating agencies and SI PPS.
  • Track and report outreach, engagement, and outcomes to SI PPS on a quarterly basis.
  • Participate in biweekly case conferences to review outreach and engagement activities.
