Requirements
  • Grade 12 education.
  • A Community Support Worker Diploma or equivalent.
  • At least 2 years of experience in community social services or health care.
  • Current Emergency First Aid/CPR and Food Safe certificates.
  • Behavior management training.
  • Ability to communicate effectively, verbally and in writing, with team members, professionals, families, and community members.
  • Non-verbal communication skills.
  • An unrestricted Class 5 driver's license; a Class 4 license is required when driving a commercial company vehicle.
  • A satisfactory driver's abstract and a reliable vehicle with $2,000,000 third-party liability insurance.
  • Criminal record clearance for children and vulnerable adults.
  • Moderate computer literacy.
  • Eligibility to work in Canada.
Responsibilities
  • Ensure the supported person's rights and choices are respected and advocated for as needed.
  • Interact effectively with people receiving support.
  • Empower the supported person to be a full citizen of their community.
  • Coordinate, implement, and facilitate the person's participation in community-based social, vocational, and volunteer activities.
  • Foster positive relationships with the person's family members, professionals, co-workers, and community members.
  • Support families in maintaining their home environment with basic housekeeping and household management skills.
  • Participate in developing, implementing, and documenting the Individual Service Plan.
  • Record the supported person's daily activities and personal health information in the case management database.
  • Apply an understanding of behavior management principles and strategies, including mental health needs.
