Requirements
  • Graduation from an NLN-accredited school of nursing and a current license to practice as a Registered Nurse in North Carolina.
  • A current Registered Nurse license in North Carolina.
  • A valid driver’s license and an insured automobile meeting state or organizational requirements.
  • Current American Heart Association CPR certification.
  • At least one year of clinical experience.
  • Working knowledge of Microsoft Word and email.
  • Typing speed of 30 words per minute.
  • Basic technical or medical knowledge of home health billing requirements.
  • The ability to work independently and effectively as part of an interdisciplinary team.
  • The ability to perform intermittent walking, standing, sitting, lifting, and patient-support activities.
  • The ability to concentrate on large volumes of paperwork and data entry, handle deadline pressure, exercise judgment, and independently organize and prioritize workload.
  • Emotional and mental stability and stamina.
  • Keen observation skills.
  • Availability for rotating on-call services, including some weekends and holidays.
Responsibilities
  • Initiate hospice services with patients and complete initial assessments of patient and family or caregiver needs.
  • Provide skilled and direct nursing care to hospice patients, including pain management, symptom relief, palliative interventions, and emotional support.
  • Develop, initiate, implement, and revise individualized hospice plans of care in collaboration with the interdisciplinary team, patient, and family.
  • Educate patients, families, and caregivers about hospice benefits, admission forms, regulatory requirements, disease progression, prognosis, medications, pain and symptoms, oxygen safety, hospice philosophy, and care of the terminally ill.
  • Communicate with attending and hospice physicians about patient needs, treatment response, and changes in condition, and obtain physician orders as required.
  • Coordinate holistic physical, psychosocial, emotional, spiritual, and bereavement services with the interdisciplinary group and family or caregiver.
  • Perform ongoing patient assessments and report pain, symptoms, and other findings to the hospice physician and interdisciplinary team.
  • Maintain accurate and current medical records, including assessments, care coordination notes, communications, visits, phone calls, physician orders, and symptom-management outcomes in the electronic medical record.
  • Supervise assigned LPN and CNA staff in accordance with Medicare guidelines and agency policy.
  • Ensure necessary equipment, medical supplies, medications, and other services are available to support patient comfort and care.
  • Report abuse and neglect in accordance with state laws, regulations, and organizational policy.
  • Provide medication review and reconciliation in the electronic medical record.
  • Participate in 24/7 on-call nursing services, including the assigned rotation.
  • Participate in quality assessment and performance improvement teams and activities.
  • Schedule work to reduce driving time and mileage and avoid duplicate trips.
  • Order and manage medical supplies, durable medical equipment, care boxes, bags, and electronic devices in accordance with agency procedures and cost-containment practices.
  • Maintain a work schedule that supports team performance and assist team members in completing assignments.
  • Maintain collaborative relationships with hospice providers, long-term care facility personnel, patients, families, caregivers, and other members of the interdisciplinary team.
