Requirements
  • A Bachelor's degree in a Human Services field, or an Associate's degree in a Human Services field plus two years of residential experience, or a high school diploma/GED plus four years of full-time residential experience.
  • A clean New York State driving record is required.
  • Must be at least 18 years of age.
Responsibilities
  • With assistance from the Program Director, develop the overall Residential Service Plan with each resident and the resident's support network by identifying the individual's needs and desires.
  • Provide teaching, monitoring, and recovery-oriented supports in accordance with Office of Mental Health 595, including assertiveness and self-advocacy training, community integration and resource development, daily living skills, health services, medication management and training, parenting training, rehabilitative counseling, skill development, socialization, substance abuse services, and symptom management.
  • With assistance from the Program Director, ensure comprehensive, person-centered planning for admission, ongoing recovery-oriented services, and discharge, including completion of admission notes and compliance with New York State Office of Mental Health standards.
  • Conduct quarterly reviews of each resident's Residential Service Plan and revise the plan when necessary.
  • Observe, monitor, document, and report resident behaviors and progress in accordance with agency standards.
  • Maintain accurate and timely resident files, including progress notes, updated service plans, psychiatric and medical records, and other pertinent information.
  • Schedule annual physical, eye, and dental appointments; arrange transportation when appropriate; and follow guidelines when a resident declines or does not follow through with medical appointments or recommendations.
  • Work collaboratively with community service providers to ensure continuity of care and make appropriate referrals, including mandated linkages such as Mental Health Court, Assisted Outpatient Treatment, and probation.
  • Serve as the key liaison between residents and their clinical providers, contacting clinical providers at least monthly to monitor residents' mental health and well-being.
  • Help residents plan purchases of clothing and other necessities.
  • Work with residents to encourage healthy family and community connections and develop new supports and interests.
  • Work with the Intake Director to provide prospective residents with tours and lunch visits as part of the admission process.
  • Maintain resident confidentiality and demonstrate a caring, positive attitude toward residents, staff, guests, and service providers.
  • Safely transport residents when needed.
  • Follow safety rules and regulations for residents, staff, and self.
  • Perform other duties necessary for effective program operation.

Pan-American Square Apartments Wins Community Impact Award

Pan-American Square Apartments wins Community Impact Award. SWBR Company is pleased to announce that DePaul Pan-American Square Apartments has been recognized with the 2025 Community Impact Award by the Buffalo/WNY Chapter of the American Institute of Architects. This award honors projects that make a meaningful difference in their communities through thoughtful, inclusive design. Completed earlier this year, Pan-American Square Apartments transformed a former rail corridor into a 150-unit affordable and supportive housing community in Buffalo, NY. Designed to Passive House standards, the development provides energy-efficient homes and essential services for seniors and individuals experiencing homelessness or mental illness. More than housing, it is a catalyst for community revitalization, delivering stability, sustainability, and hope for Buffalo's most vulnerable residents. Jury Comment: The integration of community-based programs and sustainable design at the Pan-American Square Apartments struck the jury. Exemplifying the criteria of the Community Award, the Pan-American Square Apartments took a holistic approach to senior living and supportive housing that will have a lasting positive impact on the WNY community. It is clear that this project will uplift and provide stability for the most vulnerable members of its community and the design team took every opportunity to maximize resources available to its residents. SWBR provided architectural, interior design, structural engineering, landscape architecture, and graphic design services for the project. The awards celebration was held Thursday, November 6 in Buffalo, NY.
