Requirements
Hold a current unencumbered Registered Nurse license.
Have a minimum of one year of experience as an RN in a medical-surgical or acute-care setting.
Be computer proficient in typing and various programs, including experience with electronic medical records.
Possess and maintain current CPR certification if required by the state.
Have a current driver's license, vehicle insurance, and access to dependable transportation.
Responsibilities
Respond to clinical referral information, inquiries, and requests.
Partner with the Clinical Director to assess clients' eligibility and suitability for hospice services.
Provide direct hospice nursing care and support patients' comfort and quality of life.
Deliver nursing care using established principles and techniques to ease pain and support patients.
Educate patients and families and encourage their participation in developing personalized care plans aligned with their goals.
Share responsibility for call duties.

Hospice of Chattanooga is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.
Schedule: Monday - Friday, 8 am - 4:30 pm
Territory: Grundy County
In this rewarding role, you’ll deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. Qualifications
Hold current unencumbered license as a Registered Nurse.
Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
Must be computer proficient in typing and various programs, including background in EMR.
Possess and maintains current CPR certification if required by state.
Current driver's license, vehicle insurance, and access to dependable transportation.
$70,000 - $78,000 per year (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
