  • Consistently demonstrate the ability to assess, plan, implement, and evaluate nursing care for patients across specific age groups.
  • Anticipate patient needs and resolve patient concerns while evaluating and reassessing interventions for cardiac, pulmonary, and SET (PAD) patients.
  • Plan and implement individualized exercise programs in accordance with AACVPR and American Heart Association guidelines and physician recommendations.
  • Plan patient discharge in collaboration with the health care team to ensure timely discharge.
  • Use a proactive approach to patient care and respond effectively to changing patient conditions.
  • Demonstrate effective teaching skills and assess patient and family learning needs throughout the program.
  • Evaluate the effectiveness of teaching and revise instruction appropriately.
  • Document patient education and patient or family responses, and advocate for patient well-being.
  • Provide nursing care to assigned patients from admission and transfer through discharge.
  • Augment initial nursing assessment data, including biophysical, psychosocial, environmental, self-care, and discharge-planning factors, with experiential knowledge to refine individualized treatment plans.
  • Document patient evaluations, departmental data, goals, patient notes, individualized treatment plans, outcomes, and exercise sessions accurately and promptly.
  • Integrate patients' and significant others' verbal and nonverbal behaviors into nursing decisions.
  • Compile, interpret, incorporate, and complete patient data into monthly individualized treatment plans reflecting physical or functional, vocational, nutritional, psychosocial, emotional, and educational needs.
  • Perform rapid observational assessments to determine changing priorities, monitor patients, report inappropriate responses, terminate exercise as needed, and update staff and physicians using Situation-Background-Assessment-Recommendation communication.
  • Graduate from an approved school of professional nursing with current registered nurse licensure in the State of New Jersey.
  • Hold current Advanced Cardiovascular Life Support and cardiopulmonary resuscitation certification.
  • Have three years of critical care or coronary care experience.
