- Certification as a Nurse Practitioner or Physician Assistant through a Family or Adult/Geriatric program.
- A valid California license and DEA license.
- Fluency in Spanish.
- Meet the specified immunization standards and provide current documentation for Varicella, Tdap, COVID-19, MMR, and Hepatitis B, plus TB test results valid within the last two years, before the start date.
- Provide comprehensive primary care, including preventive care, chronic disease management, and acute care.
- Conduct medical assessments, develop treatment plans, and coordinate referrals.
- Collaborate with a multidisciplinary team, including nurses, behavioral health providers, and care managers.
- Maintain accurate documentation in the Electronic Health Record to ensure continuity of care.
- Participate in quality improvement initiatives to enhance patient outcomes.
- Advocate for health equity and culturally responsive care.
- Experience with Epic, the Electronic Health Record system.
