Requirements
  • The role requires Registered Nurse education or qualification.
  • At least one year of experience as a registered nurse in acute care hospital, oncology, long-term care, hospice, or home health settings is required; school internships and clinical hours may be considered in individual cases.
  • A current license to practice nursing in the state of employment is required.
  • Current CPR certification is required.
  • Current automobile insurance and a valid driver's license are required.
  • The candidate must have knowledge of terminally ill patients and their families, hospice services, hospice discipline roles, community resources, general nursing practice, managed care principles, and applicable regulatory guidelines, including Medicare, Medicaid, and ACHC.
  • The candidate must demonstrate patient assessment skills, documentation and program management knowledge, and effective oral and written communication.
  • The candidate must maintain transportation for work and be able to operate nursing assessment tools, computers, facsimile equipment, a copier, and a cell phone or beeper.
  • The candidate must work in patients' homes or institutional settings and use an automobile, with potential exposure to infections, communicable diseases, odors, blood, excreta, and hazardous materials.
Responsibilities
  • Serve as the primary point of contact for hospice patients and families throughout their hospice journey.
  • Deliver and document skilled, hands-on nursing care according to each patient's individualized plan of care in the patient's home, assisted living facility, or other residence.
  • Perform comprehensive initial, ongoing, in-person, and telephonic patient assessments and reassessments, including screenings.
  • Deliver skilled nursing interventions and symptom management aligned with goals of care.
  • Develop and update individualized plans of care, including visit frequency and risk identification.
  • Collaborate with interdisciplinary teams, including physicians, nurse practitioners, licensed practical nurses, certified nursing assistants, social workers, and chaplains, to develop and refine care plans.
  • Provide emotional, physical, and spiritual support, education, comfort, and bereavement guidance to patients and their loved ones.
  • Teach patients, family members, and caregivers about disease processes, self-care techniques, end-of-life care, medications, nutrition, dietary needs, and safe caregiving.
  • Communicate changes in patient condition and signs or symptoms of decline to appropriate providers, including nurse practitioners, primary care providers, and hospice physicians.
  • Support continuity of care by recognizing and supporting home health, palliative, and hospice referrals and transitions between levels of care.
  • Maintain timely and accurate clinical records, including screening and assessment findings, physician orders, progress notes, and care plans.
  • Follow infection-control, compliance, regulatory, and policy standards.
  • Maintain effective working relationships with the interdisciplinary group and involve the team when the patient's care plan warrants broader participation.
  • Participate in on-call and patient-support activities and conduct on-call services in a clinically competent and responsive manner.
  • Support quality-improvement and scheduling initiatives to ensure consistent and efficient care.
