Requirements
  • A nursing degree from an accredited college or university, or graduation from an approved Licensed Practical Nurse/ Licensed Vocational Nurse program.
  • A current, unencumbered, active license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse in the state.
  • Ability to read, write, speak, and understand English, including technical procedures and policy and procedure manuals.
  • Ability to apply fractions, percentages, ratios, and proportions to practical situations.
  • Knowledge and skills necessary to provide care appropriate to the age-related needs of residents served.

Responsibilities
  • Direct the day-to-day functions of nursing assistants in accordance with rules, regulations, and guidelines governing the long-term care facility.
  • Ensure nursing personnel comply with facility policies, procedures, job descriptions, and the Nursing Service Procedures Manual.
  • Review department policies, procedure manuals, and job descriptions and recommend revisions.
  • Plan shift services, programs, and activities with assigned nursing staff and support personnel.
  • Prepare written and oral reports and recommendations concerning shift activities.
  • Coordinate nursing services with other resident services to maintain each resident's total care regimen.
  • Participate in the development, maintenance, and implementation of the nursing service quality assurance program.
  • Participate in facility surveys conducted by authorized government agencies when requested.
  • Review and update resident discharge plans and assist with planning nursing services for discharge.
  • Interpret department policies and procedures for personnel, residents, visitors, and government agencies.
  • Admit, transfer, and discharge residents as required.
  • Complete accident and incident reports and chart resident accidents and incidents.
  • Complete medical forms, reports, evaluations, studies, charting, and other administrative records.
  • Maintain daily census reports and submit required information to the Business Office.
  • Protect assigned user credentials, resident protected health information, and facility information systems, and report suspected violations or unauthorized access attempts.
  • Complete and file required records and charts upon resident admission, transfer, and discharge.
  • Receive telephone orders from physicians and transcribe physician orders to resident charts, Kardex, medication cards, and treatment or care plans.
  • Chart nurses' notes descriptively to reflect care provided and resident response.
  • Record diet orders and forward changes to Food Services.
  • Report discrepancies involving physician orders, diet changes, and charting errors to the Nurse Supervisor.
  • Complete transfer forms and document unauthorized discharges according to established procedures.
  • Prepare and administer medications as ordered by physicians after verifying resident identity.
  • Maintain adequate supplies of floor-stock medications, supplies, and equipment and order them as necessary.
  • Maintain accurate narcotic records and report discrepancies to the Nurse Supervisor.
  • Review medication cards for completeness, accurate transcription, and compliance with stop-order policies.
  • Notify attending physicians about automatic stop orders before the final dose is administered.
  • Dispose of drugs and narcotics according to established procedures.
  • Participate in employee performance evaluations, determine shift staffing requirements, and recommend dismissals or transfers to the Nurse Supervisor.
  • Report staffing needs and absentee call-ins to the Nurse Supervisor.
  • Develop work assignments and assist with their completion.
  • Provide leadership to nursing personnel assigned to the unit or shift.
  • Make daily rounds to ensure nursing personnel perform assignments according to acceptable nursing standards and report problem areas.
  • Ensure nursing assistants are enrolled in or have graduated from an approved training program and that nurse aide trainees are directly supervised by a licensed nurse.
  • Meet regularly with nursing personnel to identify and correct problem areas and improve services.
  • Maintain effective working relationships with interdepartmental personnel and other facility departments.
  • Review personnel complaints and grievances and make appropriate reports to the Nurse Supervisor.
  • Receive and give nursing reports at the beginning and end of shift duty hours.
  • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals.
  • Report known or suspected fraud to the Administrator.
  • Ensure unattended departmental computer workstations are logged off or protected by an automatic screen saver.
  • Inform nursing personnel about new admissions and prepare rooms for new admissions.
  • Greet and orient newly admitted residents and family members to the facility.
  • Make rounds with physicians and consult with residents' physicians regarding care, treatment, and rehabilitation.
  • Arrange diagnostic and therapeutic services as ordered by physicians.
  • Review resident charts for treatments, medication orders, diets, and other care requirements.
  • Implement and maintain nursing objectives and standards.
  • Check that prescribed treatments are administered properly and evaluate residents' physical and emotional status.
  • Coordinate social and activity programs with nursing service schedules.
  • Notify attending physicians and next of kin about resident accidents, incidents, and changes in condition.
  • Carry out restorative and rehabilitative programs, including self-help and care.
  • Inspect nursing treatment areas daily for cleanliness and safety.
  • Provide catheterization, tube feedings, suctioning, dressing and bandage changes, colostomy and drainage-bag care, blood collection, massages, range-of-motion exercises, and care for dying residents as required.
  • Use restraints in accordance with established policies and procedures.
  • Obtain sputum, urine, and other specimens for laboratory testing.
  • Take and record temperature, pulse, respirations, and blood pressure measurements.
  • Monitor seriously ill residents.
  • Check food brought by residents' families or visitors against residents' dietary allowances.
  • Ensure direct care follows each resident's care plan and wishes and that residents unable to call for help are checked frequently.
  • Meet with residents and family members as necessary.
  • Arrange transportation for discharged residents and escort discharged residents to the pickup area.
  • Inform family members of resident deaths, call funeral homes when requested, and ensure postmortem procedures are followed.
  • Develop, plan, conduct, and schedule in-service training classes for nursing service personnel.
  • Implement and maintain an orientation program for new employees on the shift, its procedures, and job duties.
  • Assist in standardizing work methods and training personnel regarding potential exposure to blood and body fluids.
  • Assist the Director in planning clinical supervision for nurse aide trainees.
  • Attend outside training, annual facility in-service training, advance-directive training, and continuing education programs.
  • Monitor personnel compliance with safety regulations and departmental policies.
  • Assist with identifying and classifying functions involving potential exposure to blood or body fluids.
  • Ensure personnel receive appropriate training before performing tasks involving potential exposure to blood or body fluids.
  • Maintain adequate personal protective equipment supplies and ensure their availability.
  • Maintain assigned work areas, resident care rooms, and treatment areas in a clean, safe, and sanitary condition.
  • Ensure personnel follow handwashing, hand-hygiene, infection-control, isolation, and personal-protective-equipment procedures.
  • Ensure personnel participate in fire-safety and disaster-preparedness drills.
  • Participate in development, implementation, and maintenance of the infection-control program.
