About the job
- Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- Hold a current state license as a Registered Nurse.
- Provide proof of current CPR certification.
- Have a current driver's license, reliable transportation, and valid and current auto liability insurance.
- Have two years of experience as a Registered Nurse in a clinical setting.
- Have strong assessment and clinical skills.
- Familiarity with infection control measures under OSHA requirements.
- Knowledge of state and federal regulations.
- Work independently and as part of a team.
- Be able to work a flexible schedule and travel locally.
- Be available for PRN emergency calls.
- Work in patients' homes in varied conditions and locations.
- Perform admissions and administer nursing care for patients of all ages in their residences.
- Coordinate care with the interdisciplinary group, patient and family, and referring agency, and assume responsibility for coordination of care.
- Assess home health patients using the Nursing Assessment and LCD criteria to identify physical, psychosocial, and environmental needs.
- Develop and document the plan of care in correlation with the assessment.
- Provide nursing care using infection control measures under OSHA requirements.
- Work with the Interdisciplinary Group to ensure patient and family needs are met.
- Communicate with attending physicians, home health physicians, and other physicians involved in patients' care.
- Submit all required paperwork in a timely manner.
- Meet productivity standards.
- Perform on-call responsibilities and provide on-call service to patients and families as assigned.
- Communicate effectively with patients and families, team members, and other healthcare professionals.
- Demonstrate professional growth and competency.
- One year of full-time home health experience; preferred but not required.
- Knowledge of OASIS; highly preferred but not required.
- Knowledge of Home Care Home Base (HCHB); highly preferred.
