Requirements
  • Bachelor's Degree in Human Services or a related field.
  • Minimum of four years of experience working with individuals with intellectual and developmental disabilities and Lifesharing.
  • Valid driver's license.
  • Clear criminal record and child abuse check in accordance with state requirements.
  • Ability to work independently and with a team, manage time effectively, and support positive approaches.
  • Experience with people with developmental disabilities and supporting family dynamics.
Responsibilities
  • Serve as the point of contact for all incoming internal and external referrals, responding to inquiries within 24 hours.
  • Handle emergency referrals, respite care, placement, and billing coordination.
  • Manage all intake steps from referral and assessment through matching and placement.
  • Continuously build a pool of available and vetted providers and respite providers.
  • Review completed and approved applications, schedule and complete home studies, coordinate visits, and finalize matches.
  • Conduct Introduction to Lifesharing Information Sessions.
  • Coordinate and gather required documentation to meet licensing and KenCrest Lifesharing standards and practices.
  • Represent KenCrest Lifesharing at service fairs, presentations, and conferences to promote the program.
  • Update and maintain referral information for KPI and Performance-Based Contracting.
  • Maintain and update referral and provider information in systems such as Evolv and Teams.
