Requirements
  • All levels of experience and education welcome

  • Proof of insurance and ownership for personal vehicles used during work duties
  • Ability to qualify for PATH's insurance coverage
Responsibilities
  • Coordinate move-in into housing units
  • Serve as a liaison between tenant and community-based case management team
  • Ensure tenant well-being and unit habitability through a regular schedule of unit inspections and wellness checks
  • Provide assistance for critical issues that impact housing stability
  • Respond to housing and tenant-related emergencies during normal business and after hours
  • Identify and secure community resources in coordination with assigned case manager
  • Gain expert knowledge of homeless populations, common barriers to housing, and overall knowledge of community resources
  • Understand the building lease term agreement and provide education to tenants and case managers on lease requirements
  • Maintain client related data tracking systems, including case notes and complete HMIS and Salesforce entries
  • Prepare case-related reports, including outcomes, successes, and challenges
  • Generate client data for reporting and understand the overall importance of data collection
  • Maintain complete and accurate documentation of service objectives and outcomes as well as other services in accordance with Federal, State, County, and PATH guidelines
  • Complete follow-up and retention services, as necessary, and provide documentation in client file
Desired Qualifications
  • Two (2) years of experience in a human services or behavior-related field
