Requirements
  • Current unrestricted New York State Registered Nurse license.
  • A minimum of 2 years of nursing experience.
  • Familiarity with Office for People With Developmental Disabilities, Office of Mental Health, and Department of Health nursing regulations and program requirements.
  • Clinical assessment, crisis intervention, and care coordination skills.
  • Communication, organizational, and documentation skills.
  • Willingness to travel throughout the five boroughs.
  • Ability to work independently in a team-based environment.
Responsibilities
  • Screen individuals being considered for admission to the Crisis Respite/Transitional Program and Transitional Respite Program as an active member of the intake team.
  • Conduct initial and ongoing nursing assessments for individuals admitted to the Crisis Respite/Transitional Program.
  • Oversee the self-medication administration program, monitor therapeutic response, and document outcomes.
  • Provide direct nursing interventions, including wound care, diabetes management, respiratory illness support, and chronic disease oversight.
  • Monitor early signs of medical or psychiatric crisis and initiate appropriate interventions.
  • Review medication administration records and records documenting vital signs, elimination, glucose readings, weight, menses, and other individually specified information.
  • Report significant issues to appropriate clinicians and team members, including medication ordering, prescription renewal or updating, and preparation of person-supported drug information sheets.
  • Monitor and manage medication storage and disposal procedures in accordance with federal and state law.
  • Develop and monitor person-specific health goals, reviewing, updating, and revising them to reflect the current status of each person supported.
  • Monitor infection control and environmental changes in compliance with Occupational Safety and Health Administration mandates and ensure universal precautions are followed.
  • Order and maintain necessary first-aid and medical supplies.
  • Remain available by phone 24/7 for consultation and basic direct care.
  • Partner with Intensive Case Management teams to address medical and behavioral health needs in service planning.
  • Deliver health education and coaching to individuals, families, and direct support staff to promote independence and wellness.
  • Facilitate hospital discharge and community reintegration through Critical Time Intervention, ensuring continuity of medical, psychiatric, and social support.
  • Coordinate follow-up care with primary care providers, specialists, and community-based resources.
  • Serve as the clinical liaison among providers, families, and program staff.
  • Participate in interdisciplinary treatment planning and case conferences.
  • Train and mentor direct support professionals on health-related tasks and emergency response.
  • Maintain timely and accurate documentation in compliance with Office for People With Developmental Disabilities, Office of Mental Health, and Department of Health regulations.
Desired Qualifications
  • Background in intellectual and developmental disabilities, behavioral health, crisis care, or community health.
  • Valid driver's license; travel to multiple program sites may be required.

OPWDD: Governor Hochul Announces Awards for 'Step-Down' Programs for Hospitalized New Yorkers Living With Mental Illness & Developmental Disabilities

For five years, AHRC New York City has partnered with NYC Health + Hospitals, and now New York State Office of Mental Health and NYS Office for People With Developmental Disabilities to enable people with developmental disabilities, autism and suspected IDD and autism, who have co-occurring mental health diagnoses, to exit unnecessary hospital stays and move into a community transition program, one with intensive clinical and case management supports.
