Requirements
  • Hold membership in good standing with the applicable regulatory body: College of Nurses of Ontario, College of Physiotherapists of Ontario, College of Occupational Therapists of Ontario, Ontario College of Social Workers and Social Service Workers, College of Dietitians of Ontario, or College of Audiologists and Speech Language Pathologists of Ontario.
  • Have at least 1 year of recent experience in community health or a related field.
  • Have knowledge of the health care delivery system and community resources.
  • Have interpersonal, communication, assessment, problem-solving, and decision-making skills.
  • Have effective time management, prioritization, and organizational skills, with the ability to work independently and cooperatively in a busy multidisciplinary environment.
  • Be able to accurately complete required documentation, reports, and forms.
  • Have a valid driver's license and access to a reliable vehicle.
  • Be proficient in a Windows environment.
  • Submit proof of COVID-19 vaccination status before the start date.
Responsibilities
  • Assess care needs, determine service eligibility, and develop individual care and service plans in collaboration with patients and their families.
  • Link patients with service providers.
  • Coordinate and monitor care plan delivery.
  • Establish a helping relationship with patients and their families.
  • Balance patient needs and choices with available resources while respecting patients' values and preferences.
Desired Qualifications
  • Experience working with diverse patient groups, such as multicultural, homeless, palliative, acquired brain injury, mental health, geriatrics, or pediatrics populations.
  • Case management experience or recent related community experience.
  • Ability to speak French or another second language.
