Requirements
  • The candidate must be at least 18 years old.
  • The candidate must have a valid driver's license.
  • The candidate must have current car insurance and vehicle registration because the position requires use of a personal vehicle.
  • The candidate must successfully pass a background check.
Responsibilities
  • Perform the tasks outlined in the individual's Individual Plan (IP).
  • Complete assigned tasks as requested.
  • Perform requested services for the individual within the units authorized in the Individual Plan (IP).
  • Deliver services in accordance with Passavant Memorial Homes' policies and procedures.
  • Provide supervision and positive role modeling at home and in the community.
  • Complete the required pre-service and annual in-service training planned by the individual or representative and facilitated by Home and Community Services (HCS).
  • Observe individuals' behaviors and changes in physical condition, and report observations accurately.
  • Provide care and supervision for the individual in activities of daily living and social and recreational activities.
  • Support the individual's safety and well-being.
  • Provide services in the individual's home and/or community and use a personal vehicle to transport the individual as appropriate.
Desired Qualifications
  • Experience working directly with individuals with developmental disabilities is preferred.
