Requirements
  • Successful completion of certified nurse practitioner program
  • Current licensure as an adult nurse practitioner by the Board of Medical Examiners of Maryland/Maryland State Board of Examiners of Nurses
  • Completion of health care cardiopulmonary resuscitation course annually
  • 3 years of experience functioning as a nurse practitioner in a clinical setting
  • Ability to operate independently and balance multiple priorities
  • Strong leadership, interpersonal, and influencing skills necessary to interact effectively with physicians, payers, regulatory agencies, staff, and other health professionals
  • Skill in using computers and personal productivity applications
  • Skill in written and oral communication
  • Ability to interact effectively with patients and families
  • Skill in reviewing, analyzing, and developing clinical summaries of patient electronic medical records
  • Broad knowledge of medical treatment practices, especially internal medicine, geriatrics, and oncology
  • Knowledge of healthcare regulatory agency standards for hospice/health care organizations
  • Knowledge of The Joint Commission and CMS principles and standards
  • Maint familiarity with Part A Hospice Medicare and insurance coverage processes preferred
  • Must maintain current knowledge about changes in state and federal regulations and reimbursement policies as it relates to nurses
Responsibilities
  • Plans, implements and monitors achievement of the programs strategic business objectives. Communicates initiatives to appropriate personnel. Identifies, implements and monitors opportunities for program growth. Participates in the development and implementation of marketing plans for assigned areas. Contributes to successful partnerships and relationship management in support of the program.
  • Oversees manpower planning, staffing and scheduling in order to deliver exceptional customer service, increase census and length of stay
  • Processes payroll and expenses for assigned areas
  • Conducts training and development for staff; facilitates orientation for new staff
  • Maintains competencies for direct reports
  • Completes and conducts 90 day and annual performance reviews
  • Management of the staffs’ performance and development to include monitoring and reviewing staff productivity, communicating job performance standards, coaching staff behaviors and facilitating training efforts
  • Responsible for the interviewing and selection of personnel for hire
  • Develops and coordinates in-service activities for selected facilities, hospitals, physician groups and hospice staff Members
  • Performs QA activities for the program. Develop standard work, policies and protocols. Review processes and establish quantifiable metrics for the management of the program with another Gilchrist Leadership and the QA department
  • Represent MHRT department within Gilchrist, GBMC, facilities and other entities as needed. Reports on MHRT issues at regular management meetings. Develop and/or review policies and procedures that may pertain to the role of MHRT team members. Maintain current knowledge about changes in state and federal regulations and reimbursement policies as it relates to nurses
  • Assures that the program meets or exceeds budgeted expectations for revenues and expenses annually
  • Establishes working relationships with the Gilchrist Philanthropy Department and assists with development of a Strategic Development plan to secure contributions for the Advanced Illness Management Program
  • As directed by management, serves as the consultant or primary care provider for selected patients seeking clinical services from Gilchrist
  • Establishes diagnoses and writes orders appropriate to the patient’s care requirements and in accordance with institutional policies and Gilchrist values and quality standards
  • Performs histories, physical examinations, and ongoing clinical evaluations at appropriate intervals
  • Performs activities to meet patient/family educational needs
  • Initiates meetings with patients, families, and health care team as necessary for the patient’s care
  • Maintains up-to-date and accurate medical and billing records applicable to care of assigned patients
  • Maintains familiarity with current general medical and hospice treatment practices
  • Maintains familiarity with applicable regulations and accreditation requirements. Recommends modifications to policies and procedures as necessary to maintain compliance
  • Performs other duties and responsibilities as assigned
