Requirements
  • An active, unrestricted Nurse Practitioner license in New Mexico is required.
  • An MSN or DNP from an accredited program is required, along with board certification as an FNP, AGPCNP, or equivalent.
  • Prior hospice experience at some point in the nursing career is required.
  • Strong clinical judgment and confidence managing complex symptom burden are required.
  • The ability to navigate emotionally sensitive conversations with empathy and clarity is required.
  • Communication and collaboration across interdisciplinary teams are required.
  • A patient-centered mindset and the ability to work independently with accountability are required.
Responsibilities
  • Conduct comprehensive palliative care assessments addressing physical, psychosocial, and spiritual needs.
  • Manage complex symptoms such as pain, dyspnea, nausea, and fatigue using evidence-based approaches.
  • Lead goals-of-care conversations, including advance care planning and end-of-life decision support.
  • Collaborate with physicians, social workers, chaplains, and other team members to deliver coordinated care.
  • Evaluate and facilitate appropriate transitions to home health or hospice services.
  • Document care plans and clinical decisions accurately and in compliance with regulatory standards.
