  • Graduate of an approved school of nursing.
  • Three years of experience in chronic disease management, care management, care coordination, utilization management, or acute clinical care.
  • Maintain a current Florida registered nurse license.
  • Current basic life support/healthcare provider certification is required, or obtain it within 90 days of hire.
  • Demonstrated organizational skills, verbal and written communication skills, and the ability to lead and coordinate activities of a diverse group in a fast-paced environment.
  • Critical thinking and problem-solving skills, and computer literacy.
Responsibilities
  • Initially and concurrently assess all patients within the assigned population.
  • Accurately screen medical necessity and submit cases for Physician Advisor review.
  • Coordinate care by considering the admitting diagnosis and medical history, current treatments, age, payment source, resources, support systems, anticipated needs, expected length of stay, appropriate level of service, special or personal needs, and other relevant information.
  • Assign an initial diagnosis-related group to determine geometric mean length of stay, while monitoring and managing length of stay and transition planning through assessment, reassessment, and application of InterQual guidelines.
  • Lead and facilitate multidisciplinary patient care conferences.
  • Manage concurrent disputes.
  • Make appropriate referrals to other departments.
  • Identify complex patients and refer them to Social Work Services.
  • Communicate the plan of care to patients and families.
  • Lead and facilitate complex case reviews.
  • Identify and document potentially avoidable days.
  • Identify and report overutilization and underutilization.
  • Ensure compliance with federal, state, local, and Joint Commission regulatory standards, including review requirements for managed contracts, Medicare, Medicaid, and campus admission and continued-stay approvals.
  • Adhere to the Utilization Management Plan.
  • Integrate national care management standards by supporting medical necessity and denial prevention through utilization management; promoting appropriate length of stay, readmission prevention, and patient satisfaction through transition management; and ensuring throughput efficiency and the right sequence and level of care through care coordination.
  • Educate physicians, patients, families, and caregivers.
  • Communicate appropriately and promptly with the interdisciplinary team and third-party payers.
  • Prioritize activities in assigned areas to focus on high-risk, high-cost, and problem-prone areas.
  • Develop collaborative relationships with patient business, nursing, physicians, and patients and families to facilitate efficient movement through the continuum of care.
  • Monitor and evaluate data, fiscal outcomes, and other relevant information to develop and implement improvement strategies.
  • Forward identified quality or risk issues appropriately.
  • Maintain positive relationships with outside and onsite reviewers and payer representatives.
  • Identify cultural, socioeconomic, religious, and other factors that may affect treatment.
  • Involve the patient’s family in treatment-plan development as appropriate, explaining procedures, therapies, systems, treatment plans, and discharge plans in terms suited to the patient’s and family’s age, development, and education.
  • Review patients’ discharge plans at multidisciplinary meetings or staffing to facilitate communication with other healthcare team members.
  • Prioritize workload to manage multiple priorities and use problem-solving skills to meet goals.
  • Enhance professional growth by participating in educational programs, reviewing current literature, and attending workshops.
  • Respect the rights and privacy of others and maintain staff-member information in strict confidence.
  • Maintain regular attendance and comply with time and attendance policies and procedures.
  • Adhere to Orlando Health’s policies and procedures, Mission, Vision and Values statement, and Code of Conduct.
  • Maintain organized and easily retrievable work records and documentation while preserving the confidentiality of data and patient information.
  • Review current literature regularly, maintain and update reference materials, and keep abreast of relevant reimbursement information.
  • Serve on committees and task forces to promote quality, cost-effective care for the patient population.

Florida Cancer Specialists & Research Institute has opened a new 31,000-square-foot facility at Orlando Health Institute Square in St. Petersburg, expanding cancer care access in Pinellas County.
