About the job
- A valid nurse practitioner or physician assistant license in the state of operations and current board certification in the applicable field are required.
- A Master's Degree in Nursing or Physician Assistant Studies is required.
- The role requires travel to and from one or more assigned facilities.
- A valid driver's license, current auto insurance, and an acceptable driving record may be required in the future depending on travel needs; the employer will provide 30 days' notice, and periodic motor vehicle record checks may be conducted.
- Perform comprehensive health assessments and develop individualized treatment plans for residents in skilled nursing, assisted living, and independent living communities.
- Diagnose, treat, and manage acute and chronic medical conditions, with a focus on preventative care and long-term wellness.
- Provide ongoing care management to improve quality of life and help residents remain healthier and more independent.
- Conduct regular facility rounds and develop relationships with residents, families, and care teams.
- Monitor changes in health status and proactively intervene to prevent complications and unnecessary hospitalizations.
- Educate residents and caregivers on disease management, preventative care, and wellness strategies.
- Partner with physicians, clinical staff, facility leadership, specialists, and family members to coordinate comprehensive care plans.
- Serve as a clinical resource within the facility and contribute to interdisciplinary care discussions.
- Support initiatives focused on improving patient outcomes, enhancing quality measures, and reducing avoidable hospital readmissions.
- Maintain accurate and timely documentation within the electronic medical record.
- Ensure compliance with applicable regulatory, clinical, and organizational standards.
- Strong leadership, interpersonal, and problem-solving skills are preferred.
- Excellent written and oral communication skills.
- Punctuality, organizational skills, and the ability to function within a team.
