  • Graduation from a federally accredited or New York State-registered college with a Bachelor's or Master's degree in Nursing or another health or human services field.
  • Two years of experience as a registered professional nurse in home health care, reviewing home health care services for medical necessity, or delivering preventive health care services; experience must have been gained within the last ten years.
  • Possession and maintenance of a Registered Nurse license issued by the State of New York.
  • Possession and maintenance of a valid license to operate a motor vehicle in New York State.
Responsibilities
  • Perform medical case management functions in accordance with established policies and procedures.
  • Review medical reports to determine the nature, appropriateness, comprehensiveness, and continuity of requested medical care.
  • Recommend special and complex medical care items and services in conformity with established departmental policies and procedures.
  • Identify and compile information on individuals unable to obtain an appropriate level of care.
  • Work closely with hospital discharge planners as needed.
  • Establish and maintain close contact with physicians and allied professional personnel, and inform them of new and revised departmental policies and procedures.
  • Review and recommend revisions to medical policies and procedures as they relate to home care and changing medical treatment needs.
  • Make on-site visits to providers to ensure compliance with departmental policy, state regulations, and the law.
  • Represent the agency at Fair Hearings or Legislative Meetings as needed.
