Requirements
  • A Bachelor's degree in Human Services or a related field is required.
  • Two years of experience with individuals with mental illness, substance abuse issues, crisis/residential/respite 24-hour centers, and/or a history of homelessness is required.
  • Working knowledge of email, Microsoft Word, Excel, PowerPoint, and the Windows operating system is required.
Responsibilities
  • Provide case management to an assigned caseload.
  • Develop and monitor individual service plans in conjunction with agency consumers.
  • Assist agency consumers in accessing needed community services, including medical, psychological, substance abuse, vocational, food, and clothing services, through referral network development, referrals, and follow-up.
  • Assist agency consumers with problem-solving and crisis intervention.
  • Accompany agency consumers to medical, psychiatric, and public benefit appointments as necessary.
  • Advocate for consumers as appropriate and encourage and support self-advocacy.
  • Identify problems in daily living skills and provide hands-on assistance and/or referrals to address them.
  • Maintain a strong working relationship with therapeutic support services to ensure quality of care.
  • Document all services in compliance with agency standards.
  • Work as a member of the social service team to develop off-site recreation, socialization, educational, and skill-building activities that promote community inclusion and integration.
  • Liaise with community-based services and groups, including civic, social service, government, and business organizations.
  • Participate in ongoing individual and group supervision, team meetings, case conferences, division meetings, and trainings.
Desired Qualifications
  • A Master's degree in Human Services or a related field is preferred.
  • Bilingual English/Spanish is a plus.
